Hi,

Apparently I submitted the IRS reports to the embassy instead of form 1040. Now they are asking me for my fiance's 2008 form 1040. What is this form? Does he need to fill out the form or does the IRS send one? I am guessing he should have the form when filing for his 2008 taxes?

Please Help!

It is the income tax form that taxes are figured on for the past year.

It is the standard US income tax reporting form. Yes, he should fill out an I-134 affidavit of support and attach evidence of his income and his IRS 1040 form with all attachments for 2008 PLUS his w-2s and 1099s. OR get an IRS transcript for that year which can be had for free. He knows what his 1040 form is. ALL Americans over age 16 know what a 1040 is.
