My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

WARNING: Phrases in this post may sound meaner than they were intended to be. Read the Adjudicator's Field Manual from USCIS

Timeline

We are in Wisconsin, very far south, just north of the Wisconsin/Illinois border. We went to Concentra - Occupational Medical Center - 358 Junction Road, Madison, WI 53717, 608-829-1888 for our medical exams. The cost for the exams is approximately $150/person (we needed 2 medical exams) depending on whether or not your immunizations are up to date and you have a record of that. In the event that you have no proof of required immunizations or you need immunizations, Concentra suggested we go to the county health department where we live to get those up to date and provide necessary documentation. Once that was taken care of, when we returned to Concentra the second time to check the TB screen, we took those new records with us, which fulfilled the documentation requirement for Concentra enabling them to complete the paperwork. The county health department provided the necessary immunizations and documentation for far less money then Concentra would have charged us. They charge $15/immunization/person for adults, and they are free for children.

The interview and biometric processing is done at the USCIS office in Milwaukee, Wisconsin, which is located at 310 East Knapp St.
