My fiancee just arrived but she didn't have any of her vaccination records with her, like DS 3025. Is this a form I can fill out myself? if so where do I download this from from? I looked on the USCIS website and couldn't find it.

I'm thinking I'll need to include copies of her vaccination records when mailing the AOS forms, right? she only has vaccination card from the clinic that's it.

I'm a bit worried now, because I don't know how to include copies of her medicals during the AOS stage. advice please.

DS-3025 is prepared by the panel physician at the foreign medical... it is not something that can be self prepared

to get a vaccination history you would need to see a civil surgeon and have them prepare the I-693 Item II Part 5

If a properly prepared & complete DS-3025 was included in the POE packet then the answer is suppose to be no...

However, you have no proof that a DS-3025 was included in the POE packet... so you have a choice... submit the I-485 with a possible expectation of an RFE (and then get the CS to transcribe the vaccinations) or have the CS transcribe it up front and include it in the original filing.

Better safe than sorry, I'll choose option 2. So what is CS and where do I get the CS to transcribe the vaccinations upfront for me? like you said, I have no clue what was in that brown envelope.

google "USCIS civil surgeon locator"... on the USCIS webpage you can put in your zip code and it will provide a list of those nearest you
