Good day all! Just have a few questions guys. Any help/response will be greatly appreciated:

1. I believe the I-693 should be submitted when I apply for AOS. Having said that, will I be undergoing the whole medical exam again as I did here in PI? I underwent the sputum smear/culture test when I had my medical exam at St. Luke's because I had a scar in my left lung since I was diagnosed of having TB before.

2. My given name in my visa includes my middle name. I think I included my middle name in my 156K form when I went for my interview. Will that cause any trouble? Ex.: my name is Juan S. Dela cruz so that makes my given name is Juan. However, in my visa, my given name is Juan Santos (Santos being my middle). Will that be a problem in the immigration?

Re your questions:

2. Just fill in the forms the same you did for the K1. It shouldn't a problem as long as it's all the same.

As per guides step 11,

I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. Regardless of if you are exempt from a medical exam, you are still required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you.

This is wrong. Please read the link I provided. A I-693 isn't ALWAYS needed.

I did not actually give advice so I am not wrong. If you read the Guide it clearly states only the vaccination supplement portion is needed not the whole I-693 if AOS is being filled within one year of K-1 medical exam.

This is wrong. Please read the link I provided. A I-693 isn't ALWAYS needed.

I would DEFINITELY not risk not sending in the I-693 these days, after the # of RFEs issued to November/December filers (some who even DID send it in), unless you don't mind a few weeks delay if you get an RFE.

I also had undergone the same sputum culture for two months with st. Lukes. Because of this, I was called by the County Health Department, pulmonay section here in Texas to undergo Tb skin test with them. I complied and was found positive of carrying TB germs. They then required me to have another x-ray taken by them. After all that, the health center's doctor told me that I have latent or inactive TB. They made me choose whether I would take Isoniazid or not. I agreed to take the medication just to make sure that the inactive Tb germ would be killed or be prevented from erupting in the future. They recorder everything and told me that they will forward all my medical record with to the USCIS.

I also went to the county health vaccination center to have my Tdap and vericella vaccine taken. Each shot cost 15USD.

I went to the Civil Surgeon to have him sign my form I-693, and he asked for my vaccination records so I gave him the DS 3025(vaccination record from st. lukes), two vaccination sheets proof from county health department and then they called the pulmonary section to fax them my medical record for my latent TB findings.

They insisted that I should still undergo medical with them (blood work)just to check for HIV, etc. Their medical plus signing the form I-693 cost 129USD.

The next day, I was handed a sealed envelope addressed to the USCIS. I included that to my I-485 application. It's been two weeks since my biometrics.

1. since you're a K1, if your medical exam was taken within the one-yr limit (and you passed since you're already here in the US, then you don't have to do it again

but you still have to submit part of the I693 form filled out by a civil surgeon, especially if you have to complete some vaccines you got from the Phils

read the AOS guide instruction and you will see that the form will be placed in an envelope and sealed by the dr.

2. your name won't be a problem, but remember this: here in the US a middle name is any second name given you (ex. mario (first) juan (second) dela cruz (last name)

they don't include their mother's maiden name as their middle name

1. since you're a K1, if your medical exam was taken within the one-yr limit (and you passed since you're already here in the US, then you don't have to do it again

Thanks. Do you have a scar in your lung? Like me, I have a scar in my lung and might not get healed anymore. And my doctor before told me that my tb skin test will always be positive because of the lung scar. That's what I am scared about. If my tb skin test turns out positive and I have a lung scar, will they still ask me to do the cuture test again? Hope not :(

And my doctor before told me that my tb skin test will always be positive because of the lung scar. That's what I am scared about. If my tb skin test turns out positive and I have a lung scar, will they still ask me to do the cuture test again? Hope not :(

I also had scar in my one of my lungs. They asked me a lot of questions related to TB symptoms and I answered accordingly. For example, they asked if I had back pain, chilling, lost of appetite, etc. I didn't feel those symptoms so after checking the result of my x-ray, they cleared me with the record of positive TB skin test with latent or inactive TB which is not communicable. Latent TB means that you are not sick with TB but there is TB germ in your body which was acquired anytime between birth to diagnosis. Don't worry sis because the doctors here are equipped well enough. TB has cure. Under proper medications and close watch of your immune system, you could live the rest of your life free of TB.

As long as you are given a visa to come to the US and your fiance loves you so much, there is nothing to worry. When you get here through K1 visa, you will need to wait for an average of 4 to 6 months before you can work or do other things that americans do here SO, even if you will undergo sputum culture, you should be fine. It is better for you to get rid of that fear by undergoing TB medications and be cleared which I believe won't happen in your case, than being chased by the fear that you might still have communicable disease that would limit you from associating with others.
