Filling out I90, some help please!

Hello all!

I'm having a little trouble filling out my I90, I've been a non conditional IR card holder since 2005. Due to an... unpleasant discovery about my US husband on entering the US when we emigrated (he came over to the UK to marry me, and we had lived there for 5 years before moving to the US), we got divorced a year and a half after moving back to the US.

During the divorce, I filed for court permission to change my last name back to my mothers maiden name, and was granted permission, however since I was also involved in buying a house at the time I never got round to legally changing my name via the bank etc despite having my old name being allowed as my legal name. After a month or two that became one of those "I'll do it next month" things and I simply didn't get around to it till now, 2 years later. In other words despite being legally able to use my old name, I haven't, but I'd like to do so now.

So anyway... there's a lil backstory for ya! On to the practical stuffs.

While I'm filing out the I90, in part 1do I put my new name or do I put the name I have been using up till now?

Also, and this is a stupid question but I'm having a Monday, for the Port of Entry do I put the airport abbreviation or the city?

I'd appreciate some help on this, I'm going to send in my paperwork at the beginning of Feb, and would like to get it right first time :)

I just filed an I-90 myself and I put down the new name. For the port of entry, I would play it safe -- airport name and city. Goodluck!
