A few questions for interview and DS-156

I am applying in Dublin, Ireland so some of this info may be embassy specific, i'm not sure.

Firstly, the medical doctor told me to take the xray in my suitcase, the receptionist insisted i bring it to the interview.. which is correct?

I would assume i just take it in my suitcase.

Should i make photocopies of everything i bring with me?

Some people seem to need an envelope with stamps affixed to it so the visa can be posted to them and others collect the visa on the day of the interview. I wasn't told i needed an envelope so is it safe to assume if i am approved i will just need to collect the visa later that day? Or should i bring the envelope with me just in case?

On the DS-156 it asks for Place of Issuance of passport and Issuing Country.

I am a British citizen and have a British passport but i am living in Ireland and had to apply to the British embassy in Dublin to get it.

I'm just a little confused about what exactly i need to write. Do i type Dublin as being the Place of Issuance and UK as Issuing Country? Or UK for both? I don't want to mess up the form =)

It also asks for a National Identification Number. Do i leave that blank as i have no idea what that is.

Should i also write 'none' for any fields that are not applicable or leave them blank? I know some people can be particularly fussy about this.

There is a question which asks Has Anyone Filed a Petition on Your Behalf? Obviously my fiance did so do i check yes? I looked at some other sites and they said to check no.

Do i need to affix a photo to the form or will they do this themselves?
