Two Way Visa Action form(DS-0221

Does anyone know how to fill out this form? It was given to my fiance when they gave her the necessary documents for her I-601 Waiver interview date in Rio at the end of January, but I'm not sure how to fill it out. It is in the checklist of documents that have to be completed for the packet, but it almost looks like it should be filled out at the interview.

Any help on this?

According to a prominent immigration attorney whose writings I've read a lot of, the CO asking for that form usually means they don't know what they are doing. It is *supposed* to be filled out by the consulate but a lot of the time they either send it to you and tell you to fill it out or tell you to find it yourself.

Her advice was to fill out what you can and submit it so that they can fill out the rest.

Rio is in the middle of changing procedure. There is some grand confusion. I just received one email saying that no documents in Portuguese need translations into English and then another one hour later about a different case saying they do need to be translated. I would fill out the form as best you can.

While it seems it should only be an internal form it is part of a checklist that Rio is just now handing to people. I am inquiring why it is on the list.
