Sign in failed.....But received Checklist Cover Sheet....Is it normal?

Good day to all,

I just wanna know,if it is normal that you sign in failed then received checklist Cover sheet?

our CR1 sign in failed last January 12,2010. We received a letter from NVC today says CHECKLIST COVER SHEET.

Their is [] symbol and X...BUT ....in checklist they put X says:

[X]--If you have to returned your original form to us...these are the item are still incorrect/missing.If you need another form,please visit http://travel.state.gov.visa/visa4354 html to download new form and and complete the instructions.

[X]--On line 30: the (sponsor) has not correctly filled up his /her own name in the blank " I________________certify........

We already response the last part they are asking and now confused what the first X means.

Anybody can help us to understand what is it mean?
