How about a sticky that people are entitled to a copy of their medical exam

The US embassy in Croatia personnel was very nice to me and they made a copy of my medical exam for me to have. I did not need it as I was a DCF/CR-1 applicant.

I now see this is often not the case for other people even though on the form itself there is a note to the doctor to give a copy of (at least) vaccination sheet to the applicant. Thus all the problems with go get another copy of medical exam or a new medical exam because your medical exam disappeared in the black hole.

Can someone put a sticky on the relevant forums so applicants know they are entitled to a copy and if are not given one to ask for one?

I'd imagine for the cost of medical we pay we should be given a copy to have.

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

I knew about that :-)

I meant for the immigrant visa applicants and those on K-1 and K-3 PRIOR to adjusting status. Often times people do not read what awaits them ahead during AOS and only then does this issue really come to light.

Kind of like:

Make sure you receive at the embassy from the interview:

K-1/CR-1/IR-1, K3 visa - check

mysterious brown envelope - check

copy of my medical exam - check

Ok, too many RFEs sent out to the K-1s (recent post from a Canadian girl). Can someone pin the info to ask for a copy of the medical or vaccination records BEFORE they leave the interview? If necessary, I'd put them in the regional forums as well since people often ask there.

I know, repeating myself but this seems to be a common RFE.

However, I am not sure this woudl solve any AOS applicant's problem with a missing medical. As it relates to the medical, the USCIS appears fairly "#######" about the "chain of control" on this document. For example, they will not accept the DS-3025 as a valid document for vaccinations if it was NOT in the POE envelope. In addition, they do not accept the I-693 that is not in a "sealed" envelope from the Civil Surgeon.

Yup, but how would they react to being sent a copy of the DS-3025 as proof that they have lost it, when people get RFEs for them during their AOS?

Could it also be useful to have to help 'encourage' a CS to see that you do not require another full medical, seeing as you have proof of having had one, the date it was carried out and the fact that it is an official USCIS form - at least, as far as I'm aware?

So maybe the people saying they got it need to provide a little more guidance, perhaps similar to the "how to get a social security number" mini-guide with links to the document which says we are entitled to a copy.

K-1:

January 28, 2009: NOA1

June 4, 2009: Interview - APPROVED!!!

October 11, 2009: Wedding

AOS:

December 23, 2009: NOA1!

January 22, 2010: Bogus RFE corrected through congressional inquiry "EAD waiting on biometrics only" Read about it here.

March 15, 2010: AOS interview - RFE for I-693 vaccination supplement - CS signed part 6!

March 27, 2010: Green Card recieved

ROC:

March 1, 2012: Mailed ROC package

March 7, 2012: Tracking says "notice left"...after a phone call to post office.

but the foreign medical is not on an official USCIS form. DS-3025/2053 are the forms... the DS = Department of State

My bad, DS it is indeed. But same point in relation to the CS.

It's frustrating that there is no consistency. I experienced the same situation as the above. But I only knew to ask because I had read ahead to the next stage in my process. I was able to obtain a copy at the POE..... but that's only because the officer I had was understanding. I know a lot more were refused.
