What form to fill out for proof of vaccinations

I don't have any vaccinations record officially but I did get them all and I can prove this via the titre blood work.

I was wondering if there is any special form that I have to take it with me and give it to my doc. so she can fill out and sign it for me.

Or would the blood work lab test results be just fine?

Any form she would fill out, I assume, would have to include dates of the vaccinations, which you wouldn't have.

I believe the blood test should suffice for your medical, and they'll fill out the form from there. On your DS-3025, the panel physician will write the date of your lab test to prove immunity.
