Does this mean that she must get the visa stamped in her passport by 3/4/2010 or does it mean that she must travel to the US by 3/4/2010?

If you have no visa there can be no travel to the US - it means the visa must be stamped in her passport.

Yes, that's what it means literally but as a practical matter, Consular officers simply extend the validity of the petition approval as long as needed, provided the case is still ongoing.

Thanks a lot both of you. I thought so, but like to double check everything. It is taking us a bit long to return Packet 3, so I became concerned about the validity period. She does not plan to actually arrive in the US until March 21, which happens to be my birtday (and her mother's birthday)!
