JUst some concerns.

1. What did you answer on the "Interview Date" on st. lukes form if you don't have any interview date yet? I;m on IR-! visa.

2. If you have relatives in the USA but you dont know their current address and contact numbers anymore.

and Also, If I write there "No relatives" on the st. Lukes form but before I had a denied tourist application at the US embassy manila, 7 years ago and I wrote a distant relative's name and address as a place where I will stay.

As of this time I don't know any relatives current address anymore and dont have any contact with any of them.Will that raise concerns? or is it just ok to write "No relatives" in the US?

I hope this will help.

If you don't have your interview date yet, say simply say "I still have yet to call USEM to confirm my interview date." I had my medical in advance and its only after the whole medical process that we were able to confirm my interview date.

1. I took my medical exam in advance so no interview schedule yet. I left it blank.

2. I don't know my relatives' contact numbers and addresses here so I also left it blank.

In your case, maybe it would be better to just put that you have a distant relative but unkown current address so you can be consistent just in case they will be able to get hold of your old file. Anyways, that's just a thought...

1. leave it blank or put "no date yet"

2. put "no relatives"

-you have to be consistent though, because in your visa application you will be asked if you applied for a visa before and whether it was denied or aprooved.

Hello all, thank you so much for the help, it gives me idea now on what to do.I will either leave it blank or will just put with distant relative and no current contact address to be consistent with previous tourist visa application.

However,when I took CFO seminar for changed name I put there" no relatives". I hope that won't be a problem.

You have no way of knowing if you still have any relatives in the US or not. If you do not have any contact with them how do you know they are even there? I would put no relatives or non that I know of. It is a question they almost always ask at the interview and sometimes they suspect that the relatives are arranging you to meet someone so you can join them.
