You need the I-693 vacc supplement unless your DS-3025 is complete. Part 1, part 2-the vacc supplement, and part 5 of the I-693. They should have given you that at your medical. Just look down in the left corner and see if it says COMPLETE. Mine actually said incomplete eligable for waiver based on age appropriate. They would not accept mine as it was ticked off not complete. I think that's where the RFE's are coming in on people who are just sending the DS-3025. If its complete I don't think there is an issue. But that's just my opinion. We are waiting now on the whole TB skin test as my medical was done in Montreal and they did the chest x-ray not the TB skin test. So we are thinking we may get RFE'd for that. The website says you must have a TB skin test, but I know people are being approved with just the chest-x-ray. The TB skin test is not expensive, $10 bucks here at the local hospital. We should have had it done before we sent the RFE back it. Its a waiting game now....

It really depends on the interviewer from what I've seen. Our interviewer told us that the I-693 was required from all applicants. However, I know that in several cases on these boards, it was in fact not. My wifes vaccination sheet was marked complete, and we submitted that with the I-485 paperwork, but still got an interview and the I-693 was requested. SapphireDreams just submitted the vaccination sheet I believe and she got approved with no interview. So there is no consistent pattern, hence why we went the more cautious route. We lucked out being in a major metropolitan area and having many options for civil surgeons. We had considered using Dr. Arnold until I had read someones post in the AoS upper forum that they received an RFE at their interview for using an out of state civil surgeon (Dr. Arnold) for their I-693.

The quote I included in my prior post was a direct quote from the USCIS site's instruction page for the I-693. It is worded quite poorly and could be taken several ways.

I realize that...I was just stating that it also says the K-1 visa only needs the vacc. supplement. Our civil surgeon didn't realize till I gave him the instructions for him to read. I mention it to another Dr. who basically told me that "they were the doctor and I didn't know what I was talking about".

Thanks for all your responses guys. I think I am going to risk it and send in the papers from Montreal. Worst case: I get an RFE.....and if it does go through without a hitch and then we don't get transferred to California, then I'll pay the fee then and get it all transcribed before the interview (if they ask for an interview).

At least sending it in as is gives me some time to also speak to my father in law who is a doctor and see if he can find me a cheaper civil surgeon. :)

I don't know if you hang around the AOS forum at all domegirl, but there have been tons of RFE's issued in November and December for not including the I-693. A lot of these, if not most were for cases transferred to California, just to put it into perspective. It seems like the likelihood you'll get an RFE for not including it has really increased in the past couple months.

With that said I risked it and was never issued an RFE. I ended up with an interview and still didn't need the I-693, but I think I got really lucky. I'm not saying you will get an RFE, just that there is a very good possibility you may. Worst case scenario is your case is delayed by a couple weeks. If you can handle that then risk it.

Hi everyone,

Does someone know where I could my DS 3025 transcribed...We called in Lubbock Texas, the doctor first wanted to do another mecical exam...after he said ok for $50 but he wants my vaccination record??? They are all in French!

So he doesn`t seem to know what is the DS 3025??? :bonk:

I don`t have money to pay an other $300 for an other medical and I don`t want to get all shots that I have had already!!! plus the extra $$$ that would cost to get all shots again! Really don`t need that!

I'm moving to Texas on March 16, going to get married on 24th...I'm getting ready now to send my AOS paper asap.

Domegirl, whereabouts in Florida are you?

I visited a doc's office in Longwood that transcribed the form for $50. They completely understood the requirements for K applicants and were extremely easy to find as well.

Don't know your location, but I'm sure there are others in the area. This was just the closest to me. I wouldn't risk it for the RFE, as there have been a ton issued over the last couple of months. I didn't think it was worth it, personally.
