I-765 item 1: Which USCIS office..Date(s)??

Howdy y'all! ^_^

Im living in Texas on K1 currently processing my AOS. Im a bit confuse on how I'll respond to item#1 on form I-765 (Employment Authorization). Does it ask about Dept of Homeland Security or Chicago Lockbox??? How bout the "Date(s)"? I have to be 100% certain ive provided all correct information they needed from me...worry-warts no no . Thank you for the help ^_^

09-30-2009 Medical exam passed

10-08-2009 Interview ...a SUCCESS!

10-20-2009 POE..Minneapolis

10-22-2009 SSN..woohooo!

12-27-2009 Got Married!!!

01-07-10 Received changed name Social Security Number

01-10-10 Mailed AOS, EAD, AP via USPS

01-13-10 AOS packet touched , January 13, 2010, 8:10 am, CHICAGO, IL 60680

01-19-10 AOS, EAD, AP NOA1

02-19-10 Biometrics Appointment

02-19-10 Biometrics done!

What you are looking at is not question #1 but part of question #11 which is asking if you have ever applied for employment authorization - if you have, what office was it, what date was it and then below that, what was the result. :star:

What you are looking at is not question #1 but part of question #11 which is asking if you have ever applied for employment authorization - if you have, what office was it, what date was it and then below that, what was the result. :star:
