According to the USCIS website (I will find the posting) is that for the DS-3025 to be effective for use for AOS, it MUST be 100% complete from a vaccination requirement, AND included in the POE packet. If the DS-3025 is not complete and is NOT in the POE packet then an I-693 Part II Item 5 MUST be acquired from a CS here stateside. The problem is that it is not certain (yet at least) that the USCIS is in fact adjudicating it this way in accordance with there own information but many reports they are NOW requiring the vaciination history to come only via the I-693.

This situation is now quite different from other members' experiences even those that did AOS this past summer....

At an Infopass this morning, the USCIS worker said the same thing many on here have noted: the DS-3025 can & does get lost between the US Embassy, POE, and USCIS.

So if someone submits just the three sections of I-693 (as instructed) and still gets an RFE, that likely means they can't find the DS-3025.

As someone on this thread experienced, the panel physician my husband went to in another country would not release the DS-3025 to him. They said they only release it to the US Embassy. I see no logic in this, but what can I do?

OP, please let us know if you contact CBP & they do have a copy of the DS-3025 to give you. I may try that too.

OP, please let us know if you contact CBP & they do have a copy of the DS-3025 to give you. I may try that too.

I wouldn't waste my time. Why would CBP have a copy of your form? They don't make and keep copies. They forward the whole envelope to CIS. They will just tell you that CIS probably lost it and there's nothing they can do about it.

To the OP,

I agree with what the secretary said, it is not common to request a DS-3025 from the panel physician. That's because they usually give you one, without having to ask. That clinic is obviously clueless as to what the procedure is.

Since you have no paperwork to prove anything, you don't really have many options at this point. Your only options are either to send the AOS without an I-693 and wait for an RFE or go to a CS and have them do a titer test (checks which vaccies she already had) and then do the ones she's missing. The CS will then fill out an I-693 for you to include with your AOS. Keep in mind that she doesn't need to do a full medical, but many CS's will try to convince you that she does. All she needs is to get the required vaccines completed and have parts 1, 2, and 5 of the form completed.

I'm trying to figure out this lame I-693 requirement quickly, but i've called a couple "civil surgeons" and the price I've gotten is 150 and 175 dollars.....so seriously anyone know what i can do in the state of UTAH? any civil surgeon's recomendations?

I'm trying to figure out this lame I-693 requirement quickly, but i've called a couple "civil surgeons" and the price I've gotten is 150 and 175 dollars.....so seriously anyone know what i can do in the state of UTAH? any civil surgeon's recomendations?

Thanks for the post, it is such a hassle calling these people, someone should write instructions on how to explain to someone what you need done as well, cause in the last office i called i was passed around to 4 different people, no one had any idea what i really needed. But somehow they knew how much it should cost.

I have been calling around sometimes to try to fill out that listing. It usually goes something like this:

Me: Hello. My husband is applying for adjustment of status. He has already completed a medical exam for the visa requirements, but he needs to have the vaccines he received transcribed by a Civil Surgeon. He has all of his vaccines already, I just need them transfered and signed off. How much will you charge me?

Then one of three responses:

Best (they know what they are doing)

Them: "Oh, that's $40"

Second Best (They don't know what they are doing)

Them: Some questions about what I want/more clarification

Me: (firmly) only transcriptions, no full exam required. (the key here is to make sure that the perception is that you know more about the process than they do - and it's true)

Them: (obviously pulling number from air) "Oh, that's $40"

Or...

Them: "Oh, it's $150"

Me: "That's quite expensive to have them copy over information. He doesn't need to have a full medical, only the transcription, as is in the instructions for the form. He has already completed a full medical overseas as part of his visa process by an approved physician, he had to get additional vaccines here, and I need them signed off by a civil surgeon."

Them: "We only do full medicals, we don't do that"

Me: "Well, I'm sorry you don't want my business. I will take it elsewhere."

I haven't gotten any of them to change their tune, but.....*shrugs*

Just a thought .. we could care how long AOS takes but EAD and AP are extremely important to us to be processed quickly as we have a some time sensitive matters at hand.

If I were to do the best we can with the AOS regarding the medical exam and the DS-3025 form (we are trying to track down a solution), at the risk of getting an RFE, can we still apply for EAD and AP and expect them to be processed without delay (60 days as stated in time lines)?

an RFE for any reason on the I-485 will "suspend" processing on any other interim benefits applied for based on the I-485 (which the I-765 and I-131 are)

Hi all ..

Thanks for the replies. I think this thread will be helpful to a lot of folks.

Here is what has transpired recently:

1- called medical doctor in Portugal. Secretary said to ask for the DS-3025 is not common and does not sound right and she was told she needs to speak with the doctor who will be in tomorrow. We are NOT happy about this road block.

2 - Called USCIS and they said I can call Customs and Border control to get a copy of the DS-3025 but they are not sure what they will say. She said we should have the I-693 in our application and if she can't get proof of the vaccinations she will need to have a new exam and have the vaccinations done again.

That is all so far.

Regarding 2) USCIS always says that, but when you call CBP they will tell you that they send all those papers to the local (to you) regional USCIS office. And then, when you go to your USCIS office, they tell you that they cannot give you any information and the only way to know what the government has on your wife is to file the FOIA form 639, which takes around two months to return.

I just went through this whole process with my wife and we ended up going to the CS. They have a translation sheet that allows them to understand records in many languages. Unfortunately my wife could not find a record for Tetanus and one of the MMR. Therefore, she had to get both right then and there and the CS gave us the sealed I-693 without a medical exam. We were charged only for the vaccines (a total of $97) and it would have been free if the Russian records had shown all of the vaccines.

That was the most fool-proof way because it was almost impossible to get form DS-3025 from the clinic in Russia. Besides, she needed the Tetanus shot anyway ... she was sick a couple of days after that, but now all is fine... hoping to clear the AOS hurdle without delays.

The stated timeline is 90 days, not 60. The typical timeframe is 60-70 days for most people. Expect a delay if you get an RFE, it's supposed to suspend everything. But a few people have reported getting their EAD/AP approved even after they received (and returned) an RFE. So it may or may not.

the key criteria is RETURNED or responded to the RFE... once you respond to an RFE the the I-485 resumes "normal" processing and the the I-765/I-131 will also resume...

Okay, I just talked to fiance as to exactly what happened.

Fiancee says that she had initial exam done. They sais she needs to go and get two vaccinations. She went to another health center and had them done and they entered the newly received vaccination information into her personal vaccination book that all Portuguese citizens are required to have as personal record. She faxed the pages of the book with relevant info to the Dr. that did the initial exam. He completed her records and the next day she went and picked up the sealed envelope to bring to her interview. So other than her Portuguese vaccination book (which is in Portuguese obviously), she has nothing.

We have no idea what the Dr. can provide or fax to us if we were to call and as. Not even sure what to as for. A DS-3025?? Do they know what that is in foreign countries??

So what next?

They are a visa-appointed physician, so I would assume they would know what a 3025 is. Why don't you phone them and ask how you can get a copy of the 3025? If they have no idea what that is, just refer to it as "the vaccination record indicating the shots you provided my fiancee during her physical exam."
