Thank you so much Fatima.So i shud just call the Drs office and ask for vaccination to be done ? what form am i sending with the package ? just DS-3025?

You're very much welcome! (I'm always happy to help out when I can because I know how stressful this whole process can be - my husband and I got so stressed over the medical and now we're hoping and praying everyday that we don't get an RFE)

Be sure that the doctor you will be seeing is a Civil Surgeon approved by the USCIS. You might want to call all the Civil Surgeons near you (you can use the USCIS Civil Surgeon locator here: https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV ) and check for rates and ask if they will do just the vaccination transcription. Some insist on doing the full medical for $350. The cheapest one we found charged us about $120 - the CS insisted on doing a physical exam and just the TST without any blood work. The CS didn't ask for more vaccinations because I had mine updated at the local health department and brought along my immunization record.

Here's a breakdown of the immigration medical exam at the Civil Surgeon I went to (just to give you an idea):

Required documents at first visit:

1. Vaccination records (required vaccinations must have documentation)

2. Passport

3. Knowledge of communicable diseases (measles, mumps, rubella or chickenpox)

SERVICES - CHARGE - SPECIAL INSTRUCTIONS

Physical Exam - $ 110.00

TB Test (PPD/TST) - $ 13.00 - Return in 2-3 days for the reading. (5mm or greater is positive)

Chest X-Ray (1 view with reading) - $ 80.00 (approx.) Required if TB test is positive

HIV-1 AB with reflex - $ 49.53

VDRL (by RPR method) - $ 16.31 - Lab work required for each person 15 and older. (You do not have to fast)

Lab draw fee - $ 11.00 - For each person if needed.

* Vaccines done by the Civil Surgeon can vary - anywhere from $50 up for each vaccine.

I had mine done at the local health department for only $15 for each vaccine (I didn't have health insurance at the time so we had to pay out of pocket)

The CS usually has the Form I-693 ready if they do a lot of immigration medical exams but bring a copy to your appointment just in case - this is the one you will need to send with your AOS packet - be sure to fill in the information about you (Part 1) but don't sign until the Civil Surgeon asks you to - the CS will give this form back to you in a sealed envelope (do NOT open it). You may still want to include a copy of your DS-3025 with the AOS packet just in case. ;) I paperclipped mine to the sealed envelope along with a copy of my immunization record from the local health department. I think the CS may have included copies of those inside the sealed envelope but I just wanted to be sure. ^__^;;

You're welcome!

AP stands for "Advance Parole" - the travel document that allows you to return to the United States after international travel while your AOS application is pending.

The K-1 process is generally the fastest - that's actually the main reason my husband and I opted for this route. He went to the U.S. embassy in Manila and asked about it and they said the K-1 was fastest. They gave him all the forms for the I-129F. :) We figured the next step (AOS) will probably take twice as long (or longer) but we're keeping our fingers crossed - praying everyday that it'll be a smooth process. ^___^

Yeah my K1 took 2 months. My Fiance filed October 10 of 2008 and NOA1 received on November 28. While NOA2 is received December 4 and the approved petition is good till March of 2009. So we decided to schedule my medical exam and interview in January of 2009. Were not in a hurry for me to be in US since my Fiance is staying in the Philippines as well. So i arrived here in US Mach of 2009.

We hired an Atty. for my K1 so maybe it makes a difference also.

But this time AOS no more Atty. they asked so much and we are financially tight right now.

Talking about the I-693 forms, I just had mine complete by the CS for vaccination they only ask 20$ to interpret my vaccination and have my undone vaccination in Local Health Office for 15$. But we found the cheapest CS after calling almost all of the CS accredited by the USCIS in my local area. So, as of now I'm praying that there is no more RFE's.

And i did the same thing with yours Fatima I inclused the Vaccination report as well. Even if the CS office said that they put a copy of it in the sealed envelope just to make sure. ;)

LOL. I'm glad I'm not the only one! I tend to be a bit obsessive-compulsive when it comes to paperwork. I must have gone over our AOS packet 10 times before we sent it out!

I checked our bank account again and the check image is up! I checked the back and found a number starting with MSC (13 digits total - 3 letters, 10 numbers) - I read from another thread that this is like a general case number - ending with 0. That person had only applied for AOS and EAD and found that 1 corresponded with the I-485 and then 4 for I-765. I tried it and it worked!

(The date on the back of the check was 011110 = 01/11/2010)

MSCxxxxxxxxx1 = I-485
