DS-3025: Has anyone ever obtained a copy later?

Timeline

The hospital where my husband had his medical done (in August) will not give a copy of the medical results to him. We got an RFE for I-693, and I'm pretty sure we'll need to show the DS-3025 to the Civil Surgeon to get it transcribed.

Has anyone been able to obtain a copy of the DS-3025 months after having the medical?

November 19, 2007 - Met

November 25, 2008 - Engaged

November 25, 2009 - Married

November 24, 2011 - Baby due!

You do not need the DS-3025 to get a transcription by a CS... you can use his original vaccination records (translated if need be).

Timeline

Those don't have the date of the medical exam performed in his home country, though.

I saw that the civil surgeon has to write on the form information about the original exam.

payxibka, nevermind. I see that the CS doesn't have to fill out that part (Section 2 part 1). Thanks for causing me to double-check.
