our RFE read different that yours; yours said

It was a yellow form that stated we had to return within 87 days. We have to complete Part 1 (Page 1), Part 2, Item 5 (Vaccination Section), and Part 5 (Civil Surgeon's Signature). It states not to include x-ray films, and to submit the letter with the medical. The I-693 has to be in a sealed enevelope.

Just curious-did you have any of your spouse's old vaccination records? My husband never had a vaccination card, and we don't have a copy of the DS-3025. I'm going to call the Dr. that performed the medical in Senegal to see what vaccinations he had to have for the embassy, but I'm not sure what to do if we can't get an official record. No RFE yet, but I know it's coming....

yeah, he brought his vaccination record from jamaica

i also emailed the place where he had his medical prior to arriving in the usa

they emailed me a copy of their records of the shots they gave him, brought both of these to the civil surgon

i emailed mobay hope explaining the situation

and i was emailed back within 24 hours

they sent their records of the shots they gave him for his immigration physical

what email address are you using? if you want ill look at the name of person who sent the records so you can ATT: an email for her

Steven called there in person & spoke to a guy. He asked if they could fax a copy of the DS-3025. They said they don't issue the medical results to anyone but the US Embassy. That's the same thing they told him when he was actually there & requested a copy.

Yesterday, I emailed them & explained the situation and only asked for the list of shots he got that day. Hopefully, they'll provide that information. If not, I may ask you for a contact.

Jawi, hope you don't mind we've continued your thread beyond your question.

We got the RFE & it requests a full medical. I will ask about it at my Infopass appointment tomorrow (and see if they can give us a copy of the DS-3025), but I did check with a civil surgeon nearby and she only charges $125 for the complete medical. We may just end up doing that, despite the redundancy.

Justucia, thank you SOOOOOOO much for this suggestion. All day yesterday, I kept checking my email looking for a response from the medical center, but nothing. Then I remembered this morning that I sent the request from Steven's email address. Sure enough, they responded with a scanned copy of the vaccination record they have on file, showing the 2 shots they gave him.

This is great as it will save him from getting those two shots again.
