Hi I am a U.S. citizen and filing for my spouse who is on H1B. I am filing for I-485 and need help in part II. Non-Immigrant Visa #

She was on a H1B that was issues in 2006. The visa was stamped on her passport when she made a return trip to India on that H1B. Then in 2009 she got a new job and a new H1B I-94 form. The new form does not have a visa stamped on it because she has not yet gone to India yet on that I-94. So do I use the VISA# from the original H1B which is expired or a different one? Lastly is the visa number located in red in the bottom right hand corner?

For consulate where visa was issued do I use just country or city and country?

Any help will be great. Thanks.
