I went to a CS in Danbury. But I found her to be a lil pricey. But whats good about her was that she only handles USCIS cases. Was very thorough with the paperworks. I even spoke to her directly on the phone before making the appointment (not a nurse) and discussed my case in some detail. That's why I chose her.

If u want her number let me know and I'll go look for it. But I would recommend to shop around first.

I suggest you use the CS locator and then call around. That is what I did here and it saved me the expense of another physical and x-ray

Use the Civil surgeon locator and call aorund. You shouldn't pay over $50. If you need shots get them at your doctors or local health dep.

If all of the vaccines were complete before the overseas medical, some people just send in that form (DS 3025? Not sure of the exact form number).

If you want to do the CS transcription, then all they should need is that (if the vaccines were complete) or the DS form + proof that you've since gotten the other vaccines.
