Upon arrival in Atlanta Immigration took my wifes medical documents including the sheet listing her vaccination status...How are we suppose to file the I-693A supplemental without this information. I have heard that others have retained this information through point of entry. I do not want to have to pay for another med exam and vaccinations for my wife and her daughter...it does not make sense and is very expensive. Any insight and help with this would be appreciated...Thanks!

Can you contact the medical facility in the Ukraine where the medical was done and have them send you another copy of the vaccination sheet DS-2035?

Unfortunately, Yes you have to get the medical done. I did not think my wife needed to submit her medical but later we recieved the RFE so as it stands, a medical is required. The other issue is that if your spouse came on K3, she did not get vaccinations and they need the vaccinations. If she came on K1, she probably got vaccinated and you can avoid the vaccination expense. Another thing is dont be fooled by the doctors office saying it is not covered by insurance. IT IS COVERED BY INSURANCE. Without insurance it can ve expensive $ 350 approx

My wife is from Ukraine also. We did not have her I-693A after her point of entry, I don't know if she never had it OR if the POE kept it. I suspect she never had one.

Her parents went to the local hospital and got her records and then had them translated into English (someone in Ukraine did the translating for $10.) Her parents mailed us the records. I started calling civil surgeons until I found one that knew what an I-693A was and would fill it out without a complete physical. The first couple doctors had no clue a K1 could use just an I-693A and they refused to do one. I finally found someone who knew what we needed. We paid the that doctor $15 cash and he filled out the form, sealed and stamped it, and off we went.

She does NOT have to have a full medical again unless your k1 medical is over a year old, or is close to over a year old. (K1's are specifically exempt so says the I-485 instructions)

She does NOT have to get the vaccinations again if she has them, just get the records and get them translated and then moved to a I-693A by a USCIS approved doctor.

If she entered by K1 and her K1 physical was less than a year ago she does NOT have to do the medical again. This is clearly stated in the I-485 instructions.

The medical clinics in Kiev don't do the DS-2035. My wife never had to show her vaccination records to get her K1, she just had to do a medical exam. (No shots either)

Hey Ken and Inna,

If your fiance came on a K1 you DO NOT need to have another medical exam done!! They are supposed to take your paperwork at the POE so what my husband did was he called the doctor's office that his exam was completed at and asked them to fax his DS-3025 form. You can also do this if you still have the office information. Ask them to either mail it or fax it to you, my husband had it in a matter of minutes!! :D We then went to a civil surgeon here and it only cost $25 for them to look at the DS-3025 and transfer it on to the I-693A form! You shouldn't have to have anything translated and you definitely don't have to have another medical exam. If the doctor won't give you this form then you might need to take other measures! Good luck if you need anymore help just let me know!!

I will say this though Ken...do call around. It took me calling around to find a civil surgeon who would fill out the I-693A for Joel without doing another medical. Some civil surgeons reserve the "right" (so they say) to require another medical. I found this out from a civil surgeon in the Atlanta area. She wanted to charge us $300 in addition to filling out the vaccination supplement. When I informed the secretary that a K-1 does NOT have to have another medical if the first one was done within a year of entry, she said she wasn't concerned about what the requirements were, only what her employer requires. When I asked why, I was rudely informed that Dr. Shah pays her salary and she is not going to risk it by asking why...and NO I cannot ask the dr why either...it is her right to do as she pleases. Pfffft!

We found another civil surgeon (in the ATL area) who did what we needed for $50. So it pays to call around.

I had the same or similar experiences at 7 of the 8 doctor's offices I called. Finally I found one (the last on the list ;) ) who was willing to fill out the supplement for $ 65. So, yes.....do make sure to call around, especially if the first one or two doctors aren't helpful :(

Like others have said, call around. I was given quotes from $60 to $300. Several insisted that another medical had to be done. I find it amazing sometimes how much misinformation is out there when you call each of the medical offices.
