Hello all,

I'm trying to get things sorted out for the AOS.

I've seen there is a new I-693 so I proceeded in reading the instructions again and this has me confused :

"If the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical, examination report, you will have to have the vaccination report completed by a designated civil surgeon."

My vaccination record was properly completed and included, I don't need any vaccines or follow-up, so the above instruction leads me to believe I do not need to complete the I-693 or am I missing something?

You still need to file I-693 only page 1 (information about you), vaccination section (part 2, page 3) and page 5 part 5 (Civil surgeon certification). All these must be in a sealed envelope from the designated surgeon. The cheapest way is Doctor Arnold who you can call and get everything done for 30 $ via fax.

You can file only vaccination part only if your medical examination for US visa was less than 1year ago, otherwise it is expired and you need to get it done and file a complete I-693.

Not everyone needs 693 some do some dont. My case was transfered to CSC and never needed an 693 form. Only the original vaccination sheet from the k1 medical I had.

My husband did not do an I-693 for his K1 adjustment of status. His case was transfered to California and he was awarded a green card with no RFE's, and no interview. He submitted a photocopy of the DS-3025 with his AOS application.

Some other VJ members (and the date of their GC approval) who did not submit an I-693 because they completed all AOS requirements prior to their visa medical and all needed shots were recorded on the DS-3025:

Note to others reading this: Just because you got a K visa DOES NOT mean you have all the immunization needed for AOS. You have to know what is required by the CDC and if your DS-3025 recorded all needed shots. If you got any immunization after your visa medical or after arriving in the US, then you will need to have the immunization section of the I-693 filled out.

IF your vaccinations are complete, you DO NOT need the form I-693. Submit only the DS 3025. I adjusted status for all three immigrants in this household without an I-693.

As mentioned, Dr. Arnold is retired.

Many VJers have been approved with using just the DS3025. My wife was one of them, but she had her medical before the CDC added more vaccines to the list. Since then it has become much more difficult to get all the required vaccinations. I see the CDC is updating the vaccination list again.

A civil surgeon can determine if you have had all the required vaccinations. The DS3025 normally has the box checked "Vaccine history incomplete."

Many November filers who even spent money for I-693 got their I-693 RFE... they even had their vaccinations here in US. There are November filers who even submitted both I-693 and the DS 3025 but still have I-693 RFE.

But it's your discreation if you will just submit the DS 3025 or submit the I-693 transcribe by the CS, it seems that there are many who got their visa just DS 3025, and you can read many AOS filers who have their RFE for I-693.

Yeah question about this.

My vaccination sheet says that I did not complete the Measels shot, but did infact finish it, he just didnt' look at my vaccination history properly. It's checked off as not age appropriate.

Should I be sending in my 3025 with a copy of my vaccinations to prove it was complete or get them transcribed properly?
