Can someone please tell us what name we use on the A.O.S. forms? Do we use her maiden or married name ?

You are in the AOS stage so you are now married. So you must use your married name.

You are in the AOS stage so you are now married. So you must may use your married name.

You may use either the maiden or married name - which ever you chose to have on the PR/Green Card. Most folks - who intend to change the last name, will use the 'new' married name on all the AOS (and related) forms. You don't have to, but now is the time if you're going to (changing it later comes with a form and yet another fee).

hello. i hyphenated my husband's last name to my maiden name so that takes care of my passport (which is still under my maiden name).
