DS 230 part 1 and DS 230 part 2 - are they different?

Hey all,

I think there is some confusion. I understand that I will have to fill out DS 230 part 1 and DS 230 part 2, however the american embassy in tokyo keep s leading me to this site: http://www.state.gov/m/a/dir/forms/ where I can only see one form for DS 230. Are there two different forms for part 1 and part 2?

any advice greatly appreciated!

It is all one form... However, the part I is filled out and signed... The second part (the one with the questions about if you have ever been a prostitute etc...) is filled out but NOT signed. That part is signed in front of the CO at the interview... So it is one form with 2 places for signatures...

The form is consists of 4 pages. Pages 1-2 is the Part I-Biographic Data, page 3-4 is the Part II-Sworn Statement.
