I have a few questions so I will break them down to make it easier for myself to get the answers to each part.

1. Part 6, my current individual income.

I changed jobs this year and received a severance package from my first employer and now have income coming from my second employer and also had 1 short employer in the middle. do I add these totals together for my current income? or do i just take my current wage multiply it by 52 weeks as I no longer work for the company that the last years income came from?

2. I-693

Does this get submitted with all of the other forms or is this a seperate form going to a different place? This form actually confuses me a lot as my now wife had to get her medical exam in Lima before going to the embassy for her interview.

3. Where do I send all of the documents for the adjustment of status? I-485,I-864,G-325A,I-765,I-131,I-693

4. Am I missing any forms that need to be submitted at this time?

5. How does she change her passport to reflect her new name?

Please read the AOS guide and instructions most of your answers are there. If you have not read the instructions and the guide you may be missing things more than your questions listed here.

The 693 is submitted with all the forms it is done by a civil surgeon in america, they transcribe the vaccinations, again if you had read the guides you would know this.

Please go spend some time reading the instructions and guides.

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

I have spent many hours reading forms and filling out all of the paperwork. This website has always been great when I have had questions; answers were given that were more than helpful. I found some of the answers on the from I-485 instructions/ guidelines. Yes I see that I need to submit many more items including more photo's of my wife, her Birth certificate, Copy of her passport page with her I-94 visa.
