I think this civil surgeon is trying to give us the runaround, saying she has to take the whole medical over here for like $145 or something. Just wanting to see if she has everything here that is required.. cause ill just send this sheet in with the AOS. And if we get an RFE at least it was filed in time?

Yeah she's trying do stick it to you as you only need the vaccinations or record transfer to the DS 693.

You can try finding another CS or lots take the CS the instructions for transfer.

Since Dec 14 Human Pap isn't required anymore.

Instructions for transfer?

When I called them they told me if she didnt have the HIV, Rpr, or TB Skin Test.. she has to take it over again? She knows she didnt have the TB skin test, just the X-Ray. Dont know about the other two, anyway.. I was looking at the required on a sheet and it looks like she has all the ones required for her age etc (shes 21 btw). Hmm.. maybe i should just find another civil surgeon or mail it to this other guy in tx, I heard he does it by mail. What do you think?

the CS will transfer her vaccination record from st. Luke's on the I-693. I had a TST and Td shot before going to my CS. she doesn't need another medical exam since hers is not over a year. I suggest you go to another CS if that one insist on another medical exam for your wife.

So she has all the requirements? Its just a matter of transferring? Yall can tell by the 3025?

I heard of some people sending it to this Dr. Arnold, some get RFE's still.

Ok if i send it in now.. with just Vac records, Im covered.. for getting it on time.. we have until End of January. Ill just have an RFE to deal with?

well i wont say that this will happen for you, my sheet looked pretty much JUST like that and I sent a copy in with my AOS and i never got an RFE and I got approved. Make sure to keep a copy with you just incase you do get an RFE that way you still have the paper and can see a CS to get it transcribed. And yea find a different CS if you can haha.

I would say it could be considered incomplete because the second dose of HPV was due Nov. 14. Since you are filing after that time, you could be required to get the second of the series since it is past due. Had you filed before the second dose was due, I think the DS3025 form would have been enough for AOS.

From Center for Disease Control:

A complete series consists of 3 doses. The second dose should be administered 2 months after the first dose; the third dose should be administered 6 months after the first dose.

I personally think I would try it and see if they ask for the second dose of HPV. You might get lucky and save money on the shot and the CS who would charge you to copy down the records onto the I-693. The downside would be an RFE which would hold you up a couple of weeks on the green card (not the EAD or AP) and having to go spend the money on the shot and CS. Do you have 2-3 weeks to spare if it saved you $100-$200?? Your choice since I'm only saying what I would choose to do.

I have heard people passing on that HPV is no longer required, but I didn't find that on the CDC website when I was looking for it earlier today. If you can verify that, other that VJ members passing it along, then you're good to go.

The second dose of MMR would most likely be waived unless she meets one of the criteria below...like living in a community with mumps outbreak. Most AOS applicants only get 1 MMR shot.

From CDC:

Measles, mumps, rubella (MMR) vaccination

Measles component: Adults born before 1957 generally are considered immune to measles. Adults born during or after 1957 should receive 1 or more doses of MMR unless they have a medical contraindication, documentation of 1 or more doses, history of measles based on health-care provider diagnosis, or laboratory evidence of immunity.

A second dose of MMR is recommended for adults who 1) have been recently exposed to measles or are in an outbreak setting; 2) have been vaccinated previously with killed measles vaccine; 3) have been vaccinated with an unknown type of measles vaccine during 1963–1967; 4) are students in postsecondary educational institutions; 5) work in a health-care facility; or 6) plan to travel internationally.

Mumps component: Adults born before 1957 generally are considered immune to mumps. Adults born during or after 1957 should receive 1 dose of MMR unless they have a medical contraindication, history of mumps based on health-care provider diagnosis, or laboratory evidence of immunity.

A second dose of MMR is recommended for adults who 1) live in a community experiencing a mumps outbreak and are in an affected age group; 2) are students in postsecondary educational institutions; 3) work in a health-care facility; or 4) plan to travel internationally. For unvaccinated health-care personnel born before 1957 who do not have other evidence of mumps immunity, administering 1 dose on a routine basis should be considered and administering a second dose during an outbreak should be strongly considered.

Rubella component: 1 dose of MMR vaccine is recommended for women whose rubella vaccination history is unreliable or who lack laboratory evidence of immunity. For women of childbearing age, regardless of birth year, rubella immunity should be determined and women should be counseled regarding congenital rubella syndrome. Women who do not have evidence of immunity should receive MMR vaccine upon completion or termination of pregnancy and before discharge from the health-care facility.

just a note for AOS only the first shot is needed you dont need to complete the sequence to be granted AOS, the rule states you need the first shot to be able to apply for aos and begin aos. Because of the time interval there is NO way you can have all 3 shots done properly during AOS. I had one shot and filed before december and was never asked and many other people have never been asked about final or second shots for HPV and yes it is no longer required at all for the process as of DEC 14th 2009.

Ok so my best bet is send in my AOS now, with just the first portion of the 693 filled out and the vacc worksheet. If they accept it I save $200, if they do not I have 30 days to get it done and sent back. The benefit is that my AOS is filed in the correct time frame, and there is possibility that I might not have to spend the $200 for the medical from Civil surgeon if they accept (Plus I get 30 more days to take care of it after RFE). Right? I should do it this way?

I'd keep shopping arround for another CS.

Close your eyes and jump if you are ready to do so! If you are feeling still nervous about this getting a 693 and waiting to file would help your nerves. For me I just filed and figure deal with it when it happens!

No, don't fill out anything on the I-693. Just mail in the AOS application with only a photocopy of the DS-3025 included with the other papers. The first page of an I-693 is useless to USCIS. It would have to be signed by a CS and sealed in an envelope for them to consider it for anything. We sent no I-693 at all because all the required immunizations were on the DS3025. We were among the first after the 2008 changes that brought up this I693 confusion to say (like Inky above), we'll deal with it later if they don't accept the DS3025. Many, many others have done the same since with no RFE.

You have to fill out the part 1 of the I-693 and sign your name if you're sending it by mail to a CS coz they will seal it when they return it back to you. But the vaccination sheet on page 3 of the I-693 has to be filled up by the CS. I noticed that my CS even put his initials on the bottom of page 3. Good luck Good luck!!

Send DS-3025. If she completed all her vaccines overseas, they may accept it, and you save some money and more importantly, the stress of having to deal with these crooked civil surgeons. And if they don't... you'll get an RFE, it's not that big a deal.

This person isn't following the plot. He isn't going to use a CS at all. He doesn't need to fill out anything on the I-693. He only sends a copy of the DS3025. That shows all her shots because she did every shot for AOS before she came to the US and is recorded on the DS 3025.
