So in the directions to i-485 it says that only the vaccination record is needed, not entire medical record. Can my wife submit the vaccination record signed by her Russian doctor (with translation, certified) or do we have to pay an additional fee to the civil surgeon to copy what we have (she has all the vaccines needed) to the I-693??

PS: I found someone's RFE, and it looks like the civil surgeon has to do the vaccination part... I thought Moscow embassy requires vaccinations before the interview...

Thank you very much, guys and gals. It sounds like even people who got everything done (vaccination-wise) in their medical overseas, still have to have their vaccine record re-done by the civil surgeon.

Is getting a copy of DS 3025 problematic? Seems like providing a copy of it with I-485 would prove that the vaccinations were properly completed...

Yes, that has been happening lately. You have to provide DS-3025 to the civil surgeon so they will know what vaccines you still need. In my case, I only submitted the sealed I-693.

Hope someone can answer your question about how to get DS-3025.

Where do we find out what shots are required to meet the K1 AOS requirements? I called up 2 different CS...they gave me 2 different answers.

The CS told me that the form that they fill out for vaccination is called "I-693 supplement"...is this correct? They said the actual I-693 for a complete physical.

I found out that it is much cheaper to go to your county health department for the immunization shots...and take the record to the CS for transcribing.

You will know if you still need additional vaccines by checking the form DS-3025. This form is the result sheet copy given to you when you had your vaccine shots from the country where you are from. I don't think k1 visa holder have different shots from another type of visa.

Yes. The surgeon will transcribe it to this form and then place it in a sealed envelop.
