Attending the interview may not be required - but there have been people who got blue slipped and had to return with their fiancee - In my opinion, if you can attend it is better to attend - better safe than sorry

I agree! Exact reason I'm going, in the event any questions/concerns come up, I'm right there. No blue sheet and coming back(I pray).

I would take a few check stubs if not around time of latest tax return just to prove you still have a job.

As far as the address goes, they are used to us from JA and probably other countries with addresses different then we are used to. We've all been there.

If not middle name, I would put he/she has no middle name or something like that. I wouldn't write None cause they may actually print that....it's been known to happen. If you leave it blank, they may think you overlooked it.
