But this certificate must be from US, or I can ask for a vaccination certificate from my country and send it?

Hopefully you have a copy of the DS-3025 form that was completed for your embassy interview. It looks like the 3rd/4th page of this pdf file:

http://bangkok.usembassy.gov/root/pdfs/medical_forms.pdf

If not, you will need your vaccination records and then to see a civil surgeon to have an I-693 completed (parts 1, 2, and 5 only, if your medical exam was done less than one year ago).

Do i really need to send a vaccinations certificate with all the papers for my adjustme of status?

Can i send it later?

Yes. The doctor you saw in Peru should have given you a copy of this form along with your receipt. It's a DS3025

They didnt give me anything....just the invoice. I will have to ask for a copy in Lima o have to put my vaccinations here again, my problem is that i wont be able to send it with all the documents right now, we dont have time to wait for that.

Try to get the DS 3025 by the time you have [if any interview] your interview or get a RFE.

People pull it off without the DS 3025 but it can be a "pain in the arm'. More shots.

So, because of not sending the vaccinations certificate, all my case will be denied? that's possible? Or i will have a RFE for sure?

I called to the clinic in Peru and they will give me a copy! My problem is TIME, so my question is if i can send a copy of the vaccinations (scan or fax) with all my other documents to file for AOS, or the clinica has to send me the original copy by mail?

I guess you could send the scanned copy, because the copy they gave us was a photocopy in Lima. The original was with our packet that we gave to the embassy. In any case, have them mail you the original in case they send you an RFE for it later on.

Good luck!
