A certified copy of your USA Passport records is $30: http://travel.state.gov/passport/services/...copies_872.html

The Privacy Act allows you to obtain copies of records in your own name and the records of your minor children. To request these records, please submit a typed or clearly printed NOTARIZED request that provides:

1. Your full name at birth and any subsequent name changes and/or the full name of your minor child or children, if you are requesting their records;

2. Your date and place of birth and/or those of your minor child or children;

3. Your current mailing address;

4. Your current daytime telephone number;

5. Your current e-mail address, if available;

6. Your reason for the request;

7. The dates or estimated dates your passports were issued;

8. Your passport numbers or any other information that will help us locate your records; and

9. A copy of requestor's valid photo identification

Certified copies of passport records may be requested. If you would like a certified copy of a passport record, the certification fee is $30.00 for the first certified copy of an individual's record and $20.00 for each additional authenticated copy of that record. Checks or money orders should be made payable to “Department of State".

All requests for passport records issued from 1925 to present should be mailed to:

Yes, MsTee. Yesterday, we requested the certified copies of Steven's birth certificate. Today, I'm requesting a police certificiate for myself, as well as a certified copy of my passport records (which I hadn't even heard of before!).

One thing I was thinking last night.... if I'm going to change my name during the next month, perhaps I should wait so that the documents will reflect my new last name.

It is very easy to do this. And, it will not take a full year, Nat.

Life's just a crazy ride on a run away train

You can't go back for what you've missed

So make it count, hold on tight find a way to make it right

You only get one trip

So make it good, make it last 'cause it all flies by so fast

You only get one trip

That sounds good. I already have a police record the fingerprinting, child abuse clearance and criminal background for school purposes.. I have to update every so often so I will update in Jan when my last name changes. I never heard of the certified copy of the passport either :blink:

Thanks for all the info! I started this process about 6 months ago and dropped the ball... and not for any good reason... I think I was just sick of paperwork! I was reading your list of required docs and I did not realize that copies of passport records were required, I thought it was just a notarized photo copy of the passport. Good thing you are so into the paperwork!

I feel you 100% on being tired of paperwork. I've probably used up about 5 trees during this process. Sorry, environment.
