Hi there wof family, I just Thank God that we already received the date for interview but my question is, when we filed the AOS last August we didn't include the form-I693 for vaccination instead we submit the Ds-3025 from St.luke.So along with the schedule interview is to bring some items including the I693 in a sealed envelop.

we are confused do we need to get this? or Ds-3025 is already vaild?

I did my medical in st.luke last april 2009 if ever we will went to civil surgeon do I need to have another physical examination again?cause i read on my records I do had completed series - they mark my MMR and put VH on my varicella, my insufficient time interval they mark my TD OR TDAP and Human papillomavirus..and it says as a result my vacinned history is incomplete.

how many months or year that a medical examination in st.luke will be valid that the uscis will accept the validity of my records?

How much would a civil surgeon may charge for it? just for estimate amount

You need to get the vaccination portion of the I-693 filled out by a civil surgeon. This is in the I-485 directions.

The medical is good for one year unless the USCIS misplaces it. To my experience all the DS-3025 state the vaccination record is incomplete. The civil surgeon should be able to tell you what vaccinations, if any , are still needed.

For just the vaccination transcription the fee should be $25 to $100 US. Finding a civil surgeon that will do the transcription without requiring a full medical is the problem. You will most likely need to call the civil surgeons in your area and ask if they will do the transcription only. Printing out a copy of the requirements stated on the directions may help. There is a topic on this forum about creating a list of civil surgeons that will do the transcription only.

Hi, we sent our AOS documents with I-693 and not not the DS but I received an RFE requesting for another I-693.

In the instruction, "k non immigrant visa holder are not required to have another medical examination as long as your I-485 is filed within 1 yr of your overseas medical examination"

"Even if a new medical examination is not required, you still must show proof that you complied with vaccination requirements. If the vaccination record (DS 3025) was not completed and included as part of the original, overseas medical examination report, you will have the vaccination report completed by a designated civil surgeon".

In my case tdap insufficient time interval, I had my MMR in the Phil. and Varicella 'VH"... Dr. Arnold representative told me that I already completed all the required vaccinations until we received the RFE from the USCIS.

Try to call Civil Surgeon designated by USCIS near in your area, https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV

In my experienced I tried to call CS near in our area and asked about the price it range from $70-$400 just for transcription. I went to the cheapest who knows about the k non immigrant holder, but the CS told me that I need another 2 vaccinations MMR and TD and the insurance paid for it.

hello. just go ahead have the ds-3025 validated by a civil surgeon. when i sent my AOS last august, i didn't have the ds3025 validated but when i received the letter for the interview, my husband and i thought it best to have it done. i paid 70usd for the doc's fee and he also charged me 95usd for the mmr vaccine, which he said has not been administered. good luck.
