I have a question about this form.. It says that if i apply for AOS with in one year from my spouses overseas medical examination he doesn't need another one... But it also says that regardless I would need to go to a civil surgeon and get it stamped and sealed.. My question is that do i need an appointment from the civil surgeon or is just on walk in basis? Thank you..

A courtesy call to a CS to get there procedure is best

YMMV

What state are you in? In many states it can often be challenging to find a willing civil surgeon who does not demand a new medical. Consult the USCIS list of civil surgeons in your state and start calling offices. Showing up without an appointment would likely be a waste of time.
