RFI for I-693 (Adjusting from K3)

Hi all, we received an RFI for the I-693. Biometrics has been completed.

The medical exam was done overseas in late Sept. In the packet I sent, I included a copy of the completed DS-3025.

The letter states:

Submit a properly completed and signed Form I-693, Report of Medical Examination and Vaccination Record (Rev. 06/06/08 or later) for the applicant on Form I-485. The medical examination must be conducted by a civil surgeon who has been designated by USCIS. The medical must be issued less than one year prior to the filing date of Form I-485.

The civil surgeon must conduct the exam as directed in the instructions to Form I-693 and in the the Center for Disease Control's Technical Instructions for the Medical Examination of Aliens in the United States. Both the medical examination and the vaccination record must be completed when required.

So do we:

1) Get the CS to transcribe the vaccination records or

2) Get a full blown medical?

-Was our overseas medical exam lost? Also does this delay processing of EAD?

Get a CS to transcribe the vac records onto an I-693. Rin had to get the whole physical done as we waited over 2 yrs to file for her AOS (to obtain 10yr GC instead of 2 yr - she was not working so no real hurry on AOS). Her children (who followed her by six months) were still within a year of their overseas medical. The CS transcribed the vac records onto the I-693 for about $40. The I-693 now has six parts, only parts 1, 2, and 5 are required if your overseas medical is less than one year ago. Our CS tried to tell us that the children required the whole thing, but I had the printed I-693 instructions with me and showed him it was not required (see link below - FAQs).

Not sure the impact on EAD as we had it completed when we filed.

Is there any way we can determine if they lost the medical (aside from InfoPass)? I'm thinking I just pay for the medical exam fee since I don't want any further complications.

Wouldn't know how you would know if they lost the medical. I needed a copy of my daughter's medical exam (for updated information on exactly which shots she had received). Contacted the hospital in Bangkok via e-mail (found their records dept on the internet) and they e-mailed me a PDF file.

So at the very worse, I would suspect you could contact the hospital, get the records, and have a I-693 completed by a CS.
