Send one check with your AOS paperwork for $1010 (made out to: "Department of Homeland Security"). This covers the biometrics.

We'd better get ours off in December, because Nik's I-94 expires on the 31st!

So far we are waiting for his appointment with the cheapest CS in Phoenix - 65$ for copying a sheet of info! I'm on a bit of an anti-CS roll these days. :D

Anyhow, good luck all!

We went this route: "If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693."

Hopefully we're square. :)

Yeah, unfortunately, his GP told him they didn't have one of the vaccines he needed, so we thought we'd get it here - that was before we really looked into this whole Civil Surgeon BS.

Cereza, there's a new form you can attach to the front of your paperwork, to sign up for email and/or text alerts.

G-1145

I wish I had known this too. Dang. The good news is that they cashed my check on 12/7/09!

Question??????

Do you get three individual case numbers (one for I485, I131 and I765), or is there just one number for the entire file? I got a number on the back of the check...but the status check online is not recognizing the number. Does it start with MSC?

You will get 3 case numbers. Add 1 to the number at the back of your check - that should be your I485 case number. Yes, it starts with MSC.

THANKS! We got them in the mail Saturday, and you are spot on about adding 1 to the number on the back of the check. The other two were not consecutive numbers with the one for the i485. We are officially waiting on the red tape process now. LOL

Check cashed!

Also, I asked the mods to add that email form (G-1145) to the AOS Guide.

We just sent our package last night --- after so many troubles. It should arrive this afternoon.

The I-693 form was a major cause of delay. The clinic where the USCIS accredited civil surgeon is, is not so familiar with the filling up of the form. They didn't complete the vaccination record, didn't put which series of shots or complete or which shot is not age appropriate, nor ticked any result, nor put my name in the applicant's name. They only wrote the date of the 4 vaccines I had and that's just it. I also had to supply them the form so all they had to do was fill it up. The thing is the CS is only there to sign. It's the staff who prepared it. Since we're paid already and we're afraid that they won't let us reimburse our payment, I ended up researching on how the I-693 form should be filled up. I showed it to the staff who worked on it and he was giving me BS. I showed him even the vaccination record page from I-693 form, I created for myself. Finally, he let me have it my way. In short, we paid $100 for the CS's signature and for them to just write in the form what I've wrote in the template. Talking about spoon feeding huh?!? Now, my problem is that what if my understanding on how it should be filled up is wrong?!? Oh well..I guess, we'll find out it we get an RFE for I-693. But I think I'm more confident now with my new I-693 that with the almost empty one, so we'll see...

Anyways, this is just so frustrating and I'm just glad it's already on it's way and now, all we have to do is wait and wait.

Goodluck to all of us in this journey!!!

duplicate post removed at the OP's request.

“...Isn't it splendid to think of all the things there are to find out about? It just makes me feel glad to be alive--it's such an interesting world. It wouldn't be half so interesting if we knew all about everything, would it? There'd be no scope for imagination then, would there?”
