Medical before having interview appt?

Howdy all.

I've suddenly gotten a fantastic deal on a flight to Manila, leaving next week. It's earlier than we had originally planned, and we don't yet have the interview appointment made. I actually am not yet able to pass the I-134 part of the interview, but should be able to demonstrate income from my business within a few weeks or maybe a month (it's a business I can do from anywhere and will keep working on while in the Philippines).

So I have questions:

1) Is it possible to be allowed to have the medical exam while I'm there in manila, without having the appointment letter? Or without even having an appointment made? We'll only be in Manila for about a week before going to Mindanao.

If not, is it possible to make the appointment and get the letter by next wednesday? My flight leaves LAX in the early early morning Tuesday(or late night Monday) and arrives in MNL at lunch time on Wednesday. She will be leaving Mindanao on Tuesday morning and checking in to our room the day before I arrive. So she would have to get the letter by Monday evening or EARLY Tuesday. Or does the letter come to me in the US?

2) How long is the medical exam result valid for?

3) Can we reschedule the interview easily?
