Effective 1st December 2009, Dr. Arnold is retiring so will no longer be transcribing vaccination records. Can anyone reccomend another Doctor who doesn't charge extortionate prices?

If you put what state your in it might help others to help you. I know I am not having much luck here in Wisconsin finding a doctor who doesn't want to do a whole medical again.

I take this to mean that you can use the DS-3025 if it was properly completed at the foreign medical...

Even if a new medical examination is not required, you still must show proof that you have complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit Part 1, Information About You, Part 2, the vaccination chart, and Part 5, the Civil Surgeon's Certification, of Form I-693 (in an envelop sealed by the civil surgeon). Please see the instructions for Form I-693 for further information.

That makes sense to me, however I had my vaccinations completed in the UK following the medical in London... So I'm back to square one!

I've started trying to compile a database of CS according to city/town, state, if they do transcription only and what they charged. I will post on the AOS board once I get the OK from Captain Ewok, so hopefully he'll be ok with it and we can start compiling a useful resource.

Hi.. If you're in Pensacola, I can give you the name of the civil surgeon who transcribed my I-693. He didn't charged me a penny.

Hi there Im assuming you had your medical in London like I did, when I applied for aos I just sent in the vaccination document they gave me and I didnt get a rfe so I would just try that.

Sadly we're almost 500 miles from Pensacola, we're in Orlando. I doubt they provide the transcript when mailed in? Well I was missing my final MMR jab so I had to go and get that done by the good ole' NHS. I don't remember recieving a form regarding the vaccinations, all I have is my vaccination history.

Hi, I just got my RFE and Dr. Arnold did my I-693, but I am not against him. They asked me to give another I-693. I am now calling the civil surgeon in our area. You can call them and ask how much, tell them that you are a K nonimmigrant visa holder and already had a medical overseas, you are not required to have another medical as long as your Form I-485 is filed within one year of your overseas medical examinations... etc. I tried to download the revised I-693 instructions.

I've contacted one doctor so far, and they seem to understand the requirements quite well. If you're able to make it out to Longwood, I can PM you their link/information. That's the only doctor I've contacted so far, but there are sooo many doctors in the area, you might even be able to find one closer to you.

There are three-four in Altamonte Springs, and at least a dozen in Orlando, not to mention some of the other smaller cities kickin' around. I've compiled the list, but have only contacted the one so far.

01/08/2010 - Applied for SSN in maiden name.

01/09/2010 - Married! Officially a Missis.

01/19/2010 - Received SSN in mail.

02/10/2010 - Sent I-485/I-131/I-765 to Chicago.

02/19/2010 - NOA dates for all applications.

02/22/2010 - Received NOAs in mail.

02/23/2010 - Applied for SSN for married name.

03/04/2010 - Applied for Florida DL in married name.

03/09/2010 - Biometrics appointment.

04/18/2010 - AP received.

04/23/2010 - EAD approved.

04/27/2010 - AOS Interview at Orlando USCIS (decision pending).

Yeah I'm going to call around tomorrow and find out the cheapest - I'm never sure what to say to them on the phone, and I have to be honest there's always an accent-barrier! Hopefully be able to sort this all out soon though, I will make sure I post the details on here regarding the Orlando area as soon as I know.

Unfortunately, once people file they don't tend to hang around here and answer questions like "Did anyone else find a civil surgeon that didn't rip them off who wasn't Dr. Arnold?" I agree, having a state by state list would be useful if we could get enough visibility to the question...

We're still looking at $165 MINIMUM in Phoenix. :(
