I just took my wife here. We went to the one in Garner. They were very knowledgeable and we were in and out in 25 min. Total cost was 150 dollars. 15 for paperwork, 25 for the transcription and 110 for the chickenpox vaccine.

I just took my husband to this facility for a full medical, transcription of vaccinations, TB test, Blood test and paperwork. $245.00 He will go back on Monday to have his TB test read and his medical exam. The staff that handles immigration is very helpful and made us feel at ease. We strongly recommend this facility.

I actually never saw the Dr. himself. The nurse just took my vitals and then did all the paperwork and just had him sign and seal. Was pretty fast. The only vaccine I was missing is the last shot for HPV, and she said I didn't need that for the requirements anymore, so now my vaccines are 100% complete. Also, I was able to schedule it in a week, doesn't seem like a crowded practice.

I spoke with Ally and she was wonderful. I did not ask about cost until I went to sign and pick up the sealed envelope with my wife. Ally was wonderful, got all of the documents filled out and explained them to us and gave us a copy and told me it was free since it was only the transcription. I am not sure if they would do it for free, but they did it correctly and were very kind.

I have called earlier to book the appointment date, and emailed the clinic my DS-3025 in advance.

Told to get another shot of MMR before the appointment date, cheaper price at local public health department.

Went to CVS Minute Clinic instead, paid $67 for the MMR shot.

On the appointment date, the I-693 form was filled up, signed, sealed in an envelope. Given a copy of the original I-693 form too.

Good and fast services, understand what I need for the I-693 :)

Just finished my I-693. I did call the following CS for medical transcriptions here in GUAM but they wouldn't give any details on phone. Others would say they don't do medical transcriptions, they want the whole physical examination who would charge you for 250$ excluding the shots. So I ended up in:

+200$ for physical examination

We ended up paying 301$ +7.69 for the blood test. Then I was lucky not to be positive on PPD or else another 99.88$ On Xray.

They did not accept my xray CD. With less options, this is the best deal you could have here in Guam.
