We paid $50 total – $20 Transcription, plus $30 for John's Varicella Titre. Picked up a sealed and stamped I-693 two days later, with proof that the Varicella was in John's system. Yay!

Other places we called insisted that we had to do an entire medical, and wouldn't hear anything else. Dr. Lohiya's office was not only VERY awesomely priced, they also insisted on us doing an inexpensive titre test first before doing a Varicella vaccine straightaway, as the vaccine is expensive. Imagine that, a USCIS Civil Surgeon who didn't want to take us to the cleaners!!!

Edited to add: It's walk-in only, no need to make an appointment! They close at 6 p.m. Monday-Friday.

The nurse is a bit of a pain, but so is this process. The man working the front desk was the nicest guy we ever met.

I cant really give this CS a great review, but she will sign off all the parts we need, for a good price. in my first visit to her, she wouldnt sign part 5, as with many other CS's.... but i went back to her a few days later with the I-693 instructions hi-lighted for the K1 case. she wouldnt even look at the instructions at first, but after alot of frustrated explaining from my part she re-did the vaccine chart and signed part 5, at no extra cost from the initial appointment which was impressive.

phew, so glad that part of this process is over (for now). i definately recommend taking the I-693 instructions and insisting the CS read the K1 specifics!!!

I live in Colorado and I went to this CS (already quoted). I just wanted to share my experience with this CS.

This is a very humble little office.

I didn't talk with the CS, but a Doctor Assistant helped me. She was on the front desk and she said they don't charge for the transcription, only for the vaccines ($130 for one MMR and Diphteria-Tetanus vaccine) unless I wanted to be vaccinated in a different facility.

I got vaccinated there, but I may say I don't recommend to be vaccinated in this facility.

My experience says: In my country, they always show you they are using a new syringe (they open the new syringe envelope in front of you).

In this CS office the nurse just brought the syringe already filled with the vaccine. I told her if the syringe was new (not used by anybody else) and she put an strange face. Then, she said "yeah, I'm using a brand new syringe". That made me feel insecure about the procedure but I got vaccinated anyway.

For future reference, if anyone goes to this CS please go for the transcriptions but don't get vaccinated here as I don't trust their way to proceed.
