I actually never saw the Dr. himself. The nurse just took my vitals and then did all the paperwork and just had him sign and seal. Was pretty fast. The only vaccine I was missing is the last shot for HPV, and she said I didn't need that for the requirements anymore, so now my vaccines are 100% complete. Also, I was able to schedule it in a week, doesn't seem like a crowded practice.

Did they complete your ds-3025 form instead of doing whole new medical? Please let me know i am going to Oklahoma soon on k-1 visa and i want to know if there is any immigrant approved physician who could complete my ds-3025 form

Dr Ann was really nice! After making phone calls in the area, as usual they all were unaware what im talking about and wanted to charge just for the full medical exam. After contacting Dr Ann, her receptionist was nice and informed about my case so i went to see them in person. She charged me $30 to fill the transcript and i had to pay additional $50 for the blood test for the missing vaccines in the transcript (which i just did not find old records back in my country for but i had them done) so totally i spent $80.

FYI from Cocoa Beach Florida. I just went to see Dr Moise for I-693 immunization transcription. I called first and they told me to walk in, bring my Immunization form and then I could pick it up at a later date for $60. So I drove 45 minutes, the person at the counter said I needed form 687 from Dept of Health. I explained to her that this was not necessary since I had a DS-3025 from Colombia. She refused my immunization form, gave me contact info for Health Dept, it was too late to go there that day. Just wanted to give anyone else going to see Dr. Moise this information so they don't waste their time

So this place does not offer transcription of the vaccinations only. They want you to do the full exam. I’ve called multiple locations of theirs including this office. Don’t waste your time if that’s all you need!

Is anyone from Anchorage? Its only 4 medical facilities here that authorized by USCIS. In first one doctor is out of state for few months, in second one appointments available only in January, the third one doing only full medical (i just need to transcribe my vaccinations). My husband is in military but this military hospital not doing any immigration paperwork. I am confused

Pleased to say I managed to find a really good doctor in LA; I had to get the full exam so I don't know if they do just vaccine transcription.

Cost: full exam inc STD tests $190, + optionally $80 for the TB test, $150 for the MMR vaccination, $85 for tdap. Those are the cash prices, there are extra fees for credit card. I didn't ask about vaccine transcription, sorry!

 

It adds up if you have to get everything, but if you already have the vaccines / can get your regular doctor to do them, then it's a pretty good deal. They're super transparent about how much they're charging for each component, they don't try to trick you into getting anything you don't need, and both Dr Jacobson and their assistant were lovely. They're also safe for trans and nonbinary people (feel free to message me if you want to ask more about that).

Just went and picked up my completed Vaccination Transcript. Can’t recommend Dr Ripa highly enough. Very friendly reception staff and great price. Definitely give them a call if you need vaccination transcription services.
