I followed the advice of someone above and called this office (among about 20+ others) and they were the cheapest that I found if you don't need vaccines. We paid $148 for the medical exam with the sealed envelope (including TB test). We ended up needing to get 2 more vaccines (MMR and Tdap), though, which would have been around $160 at this office. We discovered the Adult Safety Net program, which gives vaccines to uninsured adults. We were able to get one of them through that for $5 - 6201 Bonhomme Rd, 3rd floor, 713.780.5600. They give them at 7:30 am or 1 pm on most days M-F, but call ahead since some days they only do it at 7:30 am. The other vaccine we were able to get for $20 at Legacy Community Health, 6441 High Star Dr, Houston TX 77074, though it was a long wait.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

One of the senior Staffs was excellent & knowledgeable about K-1/DS-3025. She filled out my I-693 vaccination part without full medicals for a fee of $60/- (cash only). I went with an appointment to be safe.

We went into another office and inquired with another before visiting this thread and both of those civil surgeons refused to complete the I-693 without performing the full exam. Luckily, we found this thread and this post.

Dr. Lin's office staff in Fremont knew exactly what we needed and the staff provided us with the two remaining shots needed for the vaccination supplement. They completed and sealed the form, and gave us a copy. Terrific, A+. Very happy with this because we were afraid that we were going to have to pay for the full exam.

Just to keep anyone looking informed, we called this doctor the other day as I'd seen their details on here, however my husband had a very lengthy conversation with the receptionist who stated that he was no longer willing to do the transcription without doing a full medical 'due to legal reasons' and that she was not willing to ask the doctor if he would call us back to discuss directly. :(

I did post these CS details in another thread, thought they'd be useful here too. They were really helpful and knew what was needed.

My husband already had all of his vaccines and other requirements but they did tell him when he first got there that it would be cheaper to go to his regular doctor, get needed vaccines and just come back with the paperwork. But I was also told that by several different doctor's offices that wanted going to charge between $175 and $ 250 for the basic exam. Nice people, just a really long wait

Figured I'd update this one, I just called and the service is now $70, and they offer walk-ins.

Super fast and they know how to do the paperwork very well. Open on Saturdays! Did one vaccine and transcription within an hour.

If you are doing the medicals, it takes 2 visits to complete it.

Very good experience. Make an appointment first.

Both dr. Torres and Dr. Douglas Day's phone number have been disconnected. The Salem place that has been recommended is no longer 20dollars. They quoted me a 191USD, plus any extra vaccinations. The cheapest I have found so far in Portland is 220USD at

Where to not go:

There are NO civil surgeons in North Central Pennsylvania who will do the transcription of vaccination records without doing the whole medical. The office of Dr. Jeffrey Baker at Penn State Hershey Medical Group at Windmere Center in State College, PA will not do the vaccination worksheet. If you go there for a full medical anyway, the staff felt a bit impersonal and slightly rude (but of course, since you'll be giving them more money, it might be a whole other story).

On USCIS website, Dr. Lawrence Levinson in Altoona, PA is listed as civil surgeon with USCIS. He is NOT doing medical examinations at all anymore. In fact, he has retired.

That only leaves CS in the Harrisburg area who are willing to do that. I have an appointment with Dr. Abdulai Bukari in Harrisburg next Thursday and they accepted to only do the vaccination worksheet part. I will update this post after the appointment to let you know if it was worth a visit.

I know this Civil Surgeon has been posted before, but I wanted to reenforce how smooth and easy the process was with Dr Yagudin. He will do vaccination transcriptions without the medical, remember to bring a passport sized photo for the paperwork. There are no Civil Surgeons in Cape Cod that will do the vaccination transcription without the medical, hence our one-hour drive to get it done.

On the flip side, avoid Dr Paul Lee in East Falmouth, MA (http://www.capetravelvaccine.com).He tried to charge us $495 for a medical and vaccination transcription, when we tried to explain how the medical was not necessary (K1 visa, all done prior to entry into USA) he said he "didn't care how things were done in your country" and was very rude. Avoid. Very unpleasant.

Dr Steinburg answers her own phone, therefore cutting out the receptionist who doesn't know the rules, and is an efficient one woman operation. She knows the USCIS system inside out.

She transcribed my vaccinations for $30 all in like 20 mins. I drove hour and a half from Baltimore and it was well worth it as others in Baltimore didn't know what they were doing.

I just talked to her. You are correct. I was surprised when I heard Dr. Steinberg. We have a clean DS-3025 form and no adverse reports on it. She enquired about Flu shots. 30 for flu shot. Otherwise only 30 for I-693.

I would like to know how your experience ended - I mean you had no issues with the I-693 from USCIS.

I would appreciate if you can post a response - I am hoping you will see my post.
