I only did vaccination transcript, so I don't know their prices for anything else.

They were willing to take a scan copy of my vaccination record in Ukrainian and spent 40 minutes on the phone with me translating what it said. Not only they didn't ask for an official translation into English, but went out of their way to make sure I didn't get any extra shots I didn't need. The visit itself took 15 minutes.

I highly recommend this place!

Dr Bhupinder Dhalla of Dade City/Zephyrhills, near Tampa, Florida, no longer works full time, but the surgery at 4717 Airport road in Zephyrhills, 33542, where she practiced will still process the medicals or vaccination supplements for the AOS. As I came from the UK, I could get most vaccinations done there, just missing the Varicella. This surgery offered to do a 'Titer test' to check for antibodies as I had chicken pox as a child. Test was $21 and signing fee for the for the I-693 was only $20. They also mentioned that a full medical service was in the $500 range all in. Hope this is useful for somebody.

Had to get my medical fully redone as USCIS lost mine. She charged me $120 for everything she did. Filling out form, transcribing vaccinations, TD skin test e.t.c

I had to pay separately for blood work $30 and X ray $86 at the local hospital. Trying to claim back X ray on insurance.

Very nice lady and has done plenty of immigration cases.

Anyone in Central Jersey/Jersey shore area, I would highly recommend her.

British born naturalized civil surgeon, so he knew about the hoops we have to jump through. Friendly guy, great staff. Exam and blood work with sealed envelope for me cost $167. I was up to date on all vaccinations and had a TB exam less than a year ago so didn't need to do that. If you're in Central Virginia, definitely give this guy a call.

Hi all,

I just wanted to share about our experience with getting my husband's immunization records transcribed. He came on a K-1, and they refused to give him any vaccines during his physical in Haiti. So he got the required ones at our local health department, but we had a TERRIBLE time finding a civil surgeon who was willing to only transcribe the vaccination info without doing a whole new medical exam. I spent hours calling civil surgeons and digging through old posts on VJ until I was able to find Dr. Steinberg.

So I just wanted to add another post for others to find if they're having the same problem. Her name is Dr. Polly Steinberg, 4905 Del Ray Ave #302, Bethesda, MD, 20814. Phone is 301-907-9550. She works by herself and she'll answer the phone. We only had to wait 2 days for an appointment, and she charged us $30. Only $30 compared to $300 for a whole new medical exam that he didn't need! She even said she must be the only doctor on the east coast who understands the K-1 process. Super nice lady; we were very impressed!

Was quoted $290 for full exam. 100-200 cheaper than other places I called.

Was able to do vaccine transcription only for $50. My wife was missing a few vaccines and we were charged extra for those, total for just the vaccine portion of the I693 with the missing shots came out to $180.

Edited to note: The doctors/clinics for Denver listed on the wiki are no longer in service.
