I had the vaccination transcript done for $100. I needed 2 vaccines which I was allowed to get from anyplace I preferred, not the doctors office. I visited The Columbus Public Health Department and got MMR, Hepatitis A and Hepatitis B (this one wasn't required for my case but the doctor at Public Health Department persuaded me to get it). They were all free. I went back to visit my doctor 3 days after with the information of the administered vaccines and I got my transcript in a sealed envelope in about 10 minutes. I wasn't charged extra for the second visit.

It was hard to find a doctor in Columbus who would do the vaccination transcript only. One thing that I noticed after more than 30 phone calls to different doctors was that the further they were from downtown they were cheaper and more willing to help with my K-1 fiancé status requirements.

I did call a couple places around where we live (Seabrook, TX) and many of them charged around $150 for doctor appointment + $300-400 more for all vaccinations if needed, ouch! Then I call Dr. Nguyen's office and asked for the price. $85 for doc appt + a total of $250 for all the shots = $335 total.

Since my husband had his medical done in the Dominican Republic, he only needed the vaccines. We called around to so many CS and they were charging us from 330-400 to complete the vaccines, and some wanted to do the whole medical. We called Miguel Andujars office and they agreed to only do the vaccines, MMR, TDAP/AP, and Varicella. He sent us to a health facility in Brooklyn to get these for free (Except for Varicella, the health facility told us we had to go to another one close to there and pay $125) We called Andujar and he said not to worry because he wont need the varicella now and to just go to his office with the form that said he got the MMR and TD/TDAP Vaccine. Anyways, we went back and he transcibed the Vaccines onto the form and charged us 100. Even though it was a pain going to Brooklyn (the A train took us back and forth- total of 45 mins), i can say that his service was great and he charged us a great price. As i said, other CS wanted to charge us for each Vaccines seperately and then for consultation.

We have an expired medical thanks to the amazing speed of USCIS's NIPW method of approving green cards. For my husband and K2, the full medical needs to be redone. :( I called all over the central coast, but this is the best I found. Others couldn't touch this price! They were at minimum $250! :( For 2 people, it was worth calling multiple offices.

We paid $50 total – $20 Transcription, plus $30 for John's Varicella Titre. Picked up a sealed and stamped I-693 two days later, with proof that the Varicella was in John's system. Yay!

Other places we called insisted that we had to do an entire medical, and wouldn't hear anything else. Dr. Lohiya's office was not only VERY awesomely priced, they also insisted on us doing an inexpensive titre test first before doing a Varicella vaccine straightaway, as the vaccine is expensive. Imagine that, a USCIS Civil Surgeon who didn't want to take us to the cleaners!!!

Edited to add: It's walk-in only, no need to make an appointment! They close at 6 p.m. Monday-Friday.

The nurse is a bit of a pain, but so is this process. The man working the front desk was the nicest guy we ever met.

I can vouch for this. I went about 3 months ago. Paid about $30 or $35 (sorry memory a little blurry now) for transcription, $30 (?) for TB and about $40(?) for Varicella testing. Was told that the Varicella shot is expensive (over $100) so it's better to test if I already have it or not first. Got my shot and blood test, I went on a friday and the report is ready to pickup the next monday. Swift and painless (not literally though, the TB shot was torture for days!).

Update on this, now that they're done: We brought current TB test results for both K1 and K2 and also updated vaccine reports for K2. The total cost for his was $88. It was ready for pick up about 3 hours after the appt. K1 went this morning and he had to have the blood tests done (Syphilis), so his was an even $100. They told him that his would be ready for pick up tomorrow. Both were in and out of the office in about 40 min. I still can't believe the price, especially since others in our area were at minimum $250. Seriously, if you're near SB, don't even bother going anywhere else. ;)

NYC medicals:

I had to have another medical exam which we were very annoyed about, as being a K1 visa AOS applicant you are not required to!!!! It clearly states on the 'instructions' part of the 1-693 which is current only a few months ago. We actually told them that in our response to the rfe, and didn't get it done. Nothing came up on my medical, and vaccinations were completed etc. So, something is going very wrong with USCIS with this repeated medical request...but as I have learned, not much can be done about it without delaying further. I did some research and found an awesome civil surgeon:

Turnaround: walked in for appointment on the Tuesday, at 10 am (hour after they open) and was out of there in less than 2 hours. Lots of forms to fill out, saw one person who looked over vaccination and other forms, said all is good. Then wait again, before seeing Dr Blau, who was very nice. Very, very simple and not like the medical I had in Australia. I cannot emphasise how easy this was...I was a nervous wreck beforehand (despite knowing nothing is wrong). But they were all very nice, professional, Dr Blau and the other Dr were super friendly with a sense of humor. My husband was asked if he wanted to come in too. They took blood, and did the TB skin test, both painless (for needles!) and was outta there. Had to return in 48 hours to have TB test read, was in and out within about 70 minutes. Didn't have to see Dr Blau again, just the other Dr who got me to sign. Then waited for paperwork (picked up all at same time) and read through it. Waited again, then got called up to sign. They seal it for you etc, and that is it. Payment done on first day. cash or credit card. You are given a copy of everything for your own records, and he said they have it all on their database and said they can give copies if ppl lose them. (not sure of cost) No need to fast beforehand, no urine etc, just bloods. It is a walk in situation and they ONLY DEAL WITH GREEN CARD exams...bonus!!!! Only 3 other ppl waiting both times I went. Their website has all the info on there including cost. I am not sure what it costs for vaccinations if you need them, but it is extra.

Highly recommended. They have yelp reviews too. I will add one later..

I was missing the varicella shot from my medical from Romania and I made a blood test at the Health Department in Branson ($25). It appeared I had had already chicken pox. I made an appointment with Dr. Santiago, it all went smooth and he was very kind.

I highly recommend him!!!

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).
