Best civil surgeon in NYC!

I went to Dr. Anthony D Blau, Green Card Medical Exams yesterday. They know what they have to do as a civil surgeon!!! We didn't need to explain anything about the I-693 form and the vaccination supplement. I have done all vaccination before coming to the US, and I brought my vaccination history to the clinic so mine went very easy. I paid $60 for the vaccination supplement. The doctor and the lady at the front desk were really nice

We tried calling other civil surgeons in NYC including Dr Smith. Most of them only offer a full medical exam. Dr Smith charges way more than $50 for the vaccination supplement now.

I went to Dr. Noel Smith last Aug 3, 2012. When I called for inquiry I was told that he would charge $75 but when I went there, the secretary asked me to pay only $50 for vaccine transcription.

I called at least ten CS from New Jersey area and he is the only one (so far) who does transcription with a very reasonable cost. The other CS would charge $160-$180 (transcription only) and $200 to $300 (full medicals;won't do transcription alone.

I would like to share some info with those who live in Omaha, NE or near.

I’m adjusting from B2 visa, so I had to have full medical examination. I found this place probably most cheapest for what I needed in my area. And the doctor (Charles Stoner) was really nice and knowledgeable, he accepted all my vaccination records, that I actually translated myself (did not even look at the original!) and did not tried to make me one vaccine that was almost expired (one you have to get every 10 years), said "you will be fine, just make sure to get it in a year or so). Also I explained to him that I always have positive TB test cause I've got BCG vaccine twice, so he sent me just to chest x-ray right away, that saved me couple of days. When my blood test and x-ray result were done (in couple days), I went there just to sign I-693 form, which they gave me in sealed envelope after I signed it and also a full copy of it with result of my x-ray for my personal use in a future. That was nice!

Oh! Also I suppose they accept results of tests and chest x-ray from other places, because he asked if I have any with me here (I did not). They also provide all accintion if needed.

Diddo that. We had no records of the vaccinations in the United States and went to Dr. Torres' office without an appointment (he doesn't take appointments). We got the I693 filled out after having the minimum necessary shots done for $140 (a great deal in our book having looked into the other Civil Surgeons who had more expensive "requirements". Dr. Torres was friendly and worked out of one of those small offices where you can tell he is there because he likes his work and likes people. Genuine person, and genuine place where me had friendly conversations with others in the waiting room! Thanks for the reccomendation!

I am very pleased with Concentra Medical Center in Akron, OH. I went to this particular place, based on a few positive reviews I found on this site and I wasn't disappointed!

The appointment was set up by my husband, he experience some trouble explaining to people at the reception that I don't need a full medical exam, just the vaccination chart (AOS from K-1), they seemed clueless, but once he got to talk with Dr. Benson, the conversation went smoothly, he was very helpful and quickly verified the fee (a reasonable $43).

On the appointment day, the receptionist struggled yet again to understand what I wanted so she made me fill out and sign a tone of forms necessary for the full exam. Once I told her I didn't need to undergo a medical exam, she said it would be the doctor that would make the final call... Luckily, once I was called by a nurse, after an initial vision and blood pressure tests, she called Dr Benson. He is such a nice and friendly person! He apologized at least twice that I had to complete so many useless forms and then quickly filled out my vaccination chart. Since it was the beginning of October he gave me a flu shot ($25) and afterwards, I was given a sealed envelop + a copy for my records.

A very nice experience once you get to talk to the right person. A piece of advice for everyone: make the reception staff call Dr. Benson immediately once you show up to avoid the paperwork .

My husband wasn't given his DS-3025 copy at his doctor in Australia so we had to get it transcripted. Called around to many offices nearby but this was the only one that had even the slightest clue to what I was talking about (not needed ANOTHER medical and stuff) very friendly service, knew exactly what we needed, and packaged it up in an envelope we aren't supposed to open (just like the orignal medical info) and I was given a copy. Awesome!
