hi, question does anyone tried to move the place of their interview? just got my interview letter yesterday and its in reno nevada 6 hours away from my place... and one more when i sent my aos i only included my ds 3025 and my interview letter says i need to bring a completed medical examination(form I-693) and vaccination supplement in a sealed envelope(unless already submitted) well i assumed since i handed my brown envelope that contains my medical record taken march of this year in my port of entry all i need is my vaccination supplement right? this is so stressful, hehe... do i need to take all my follow up shot and transcribed it to the I-693 or can i just asked a civil surgeon to transcribed the shot i got in my ds-3025? as shot here in us is very very expensive, and money is tighten as xmas is coming... anyone experienced this? pls enlighten me.

thank you all...

You should send your 3025 to a USCIS civil doctor and have the info transcribed. You can do this through the mail. I used Dr. Arnold in Austin Texas. 30$ and maybe 1 week or so. My wife required 2 additional shots before they would sign. My understanding is most females immigrating here from the Phils need these shots as they are part of a series of shots and cannot all be given at St Lukes. The USCIS doctor will tell you what shots you need and you can go to your doctor or to your county health dept. to get the shots. Send the immunization receipt to the USCIS doctor who should transcribe onto the I-693 (Dr. Arnold does this). Yes unfortunately shots are high, ours was 248$ for just 2 shots! (groan)

You do not need a complete medical if you had your embassy medical (St Lukes) less than 1 year ago. Just need the immunization supplement.

Hope I helped

hello! i did the same thing with my aos, sent only the ds3025. the civil surgeon we initially contacted in seattle said that i just have to go to the pharmacy and have the lacking vaccine administered. went to walgreens and had tdap shot which caused me around 65usd. eventually when i got my letter for interview, my husband and i decided i better have the ds3025 certified by a civil surgeon. surprisingly, the civil surgeon said i still lack another vaccine, which is the mmr. so the shot was another 95usd on top of the 70usd professional fee. makes you wonder what shots have been administered in st. luke's during our medical exam. i had 2 shots there. anyways, the doc certified that i have completed my vaccine and he put the document in a sealed envelope for uscis use only.

btw, you may want to check this link for the location of uscis office in the state of nevada https://egov.uscis.gov/crisgwi/go?action=of...or.statecode=NV

I wouldn't recommend using Dr. Arnold any more. There is a recent thread where someone was denied on the grounds of having used Dr. Arnold and there are memos going around regional offices saying that he is not acting correctly where this is concerned. Do a forum search for more info, and make your own decision as to whether you want to risk it in the hope of it getting through, or end up having to either pay to appeal a denial or refile....
