I want to make sure I understand everything about this before I get anything started.

1- As a K-1 who just had her exam in October my fiancee doesn't need a second physical right? (One civil surgeon I called said she did, and the cost would be $300).

2- I printed out Reccomended Immunization schedule, and for a healthy 34 year old, she only needs the vaccinations highlighted in those columns, which should also be shown on the I-693 chart. She doesn't need ALL the vaccines shown on the I-693, just the ones that match up with the reccomended list right?

3- She can have some of these done at the local Health Department (cheaper) and take the record of it to this civil surgeon right?

4- We then take the I-693 and medical shot record to them, the complete it, certify it and give back to us right?

Sorry for all the questions, but this is not crystal clear. Any helpful suggestions for Michigan people?

1. Do not need to do physical at all. Just need to complete the vaccination if she is missing any.

2. Yes. I am 29 and I did MMR and both Varicella (I never had chicken pox) shots back home and over here I just needed Tdap (Tetanus).

3. Yes, go to Health Dept and get that done. Make sure you call in advance. Over here in NC, they only set certain days for new patients for vaccinations and it took a week to them to set me an appointment. Tdap is free at our Health Dept. You can also go to your own family physician if she has insurane that will cover it. Once done, take both the vaccination record from here and back home to the Civil Surgeon to fill out and sign. They usually have the I-693 form with them in their office.

4. You don't really need to take the I-693 to the Civil Surgeon office since they usually have some themselves but no harm printing a copy yourself. CS will fill out the forms, sign and seal them all in an envelope. Make sure you also get a copy of the I-693 that the CS filled out, just for your own record.

Michigan... hmm, my husband is from Michigan but he moved out to NC 3 years ago, before all the crisis. Good luck on your AOS!! Cheers and Happy Thanksgiving.

You are correct.

You'll probably have to call around several civil surgeons to find one that will just transcribe the vaccination data and for a reasonable cost. Also, if any vaccinations require a series, you only need to start the series to complete and submit the I-693. No need to wait until the series is complete.
