AOS from K-1: I-693 required if vacc. were completed?

We are putting together the AOS package, and I have a couple of questions:

1. I read in Ewok's FAQ post from 2007 that if all vaccination requirements were met before the visa was issued, the I-693 was not needed. However, elsewhere here I read you need to get it filled out by a CS and submit it. Which information is correct? (Since we are a bit late with filing for AOS, it would speed things up if we could skip the I-693.)

2. The only vaccination I don't have yet is a flu shot. I was thinking about getting one anyway (but wanted the H1N1, which wasn't available to the general public until now), but again, getting it before filing would slow things down (all I'm worried about with delays is having let the 90 window pass -- we are not rushed to get a work permit or AP at this point). Is the flu shot a requirement for AOS from K-1 or not?

Flu shot is not a requirement. Its more of a suggestion.

Some people send the I-693 some people dont, some people get RFE for I-693 some people dont! kind of seems like a hit or miss. I sent my AOS without I-693 and have not yet had any RFE or anything for it! But I do have one at home for peace of mind incase of an RFE. ( wasnt able to get it till after i sent aos the doctor was on vacation)

take the 693 form to your civil surgeon have them fill out the form sign it seal it and send it to uscis right away.

We are on a K1 visa.

what info are required for the CS to fill out the I693?

The medical report was kept at the POE...so we dont have anything in our hand. Are we supposed to get the medical back in the mail?

We do have the vaccination record from vietnam, that require one shot in another month....can we just take this slip and get the last shot?

I took my last remaining vaccines and TST result went to the CS with the record and he filled up pages 1, 3 and 5 of the I-693. I don't think we will get the medical report that we had submitted on our POE in the mail. Hope this helps.

In the AOS guide, it mentioned the DS-3025.

My wife does not have a DS-3025. How do we go about getting one?. She does have the IHO vaccination record from Vietnam.

this is the vaccination paper you got in vietnam bring that to the CS and get him to copy it to the 693.
