I have all my AOS paperwork together and when we get back from Vegas next week I will mail it off. I am having one problem though. I called 3 different civil surgeons here in Wisconsin to try and make an appointment for my I-693 (vaccination) portion and they all said NO, if I want the I-693 signed the full medical must be done, not just one portion. I told them to look at the instructions for the I-693 form that are directly on the government immigration website part III, that clearly states I do not need a medical as I am filing my AOS within one year. Part 1,2 vacc section and 5 only need to be completed. NO WAY can I get this done without them performing a full medical. SO question, does anyone know any doctors in Las Vegas that is aware of what needs to be done? Anyone?? Thanks.

Why not do as many VJers do and call Dr. L E Arnold in Austin, Texas. He will do the I-693 through the mail for a low cost. Call and leave a message, 512-444-3131, and one of his workers will call you back.

4/3/14 - NOA1 receipt date

4/4/14 - check cashed

04/29/14 - biometrics date

07/01/14 - interview date

xx/xx/xx - Oath Ceremony

I just called another civil surgeon office, this time in Green Bay, WI and same thing, complete medical and they will only accept vaccination records that are military issued, or issued my a US civil surgeon that was performed in the United States.....OMG....I am about ready to tear out my hair. edp333 I will try what you suggested.

You already did your vaccination and you just need a CS to transcribe it to the I-693? Am I correct? And Dr Austin from Texas would do this service for a small fee? And can Dr Austin do anything if you don't have any vaccinations done before.

I have the DS-3025, don't quote me on that, but I think that's what's on the corner of the paper, from Medisys clinic in Montreal when they did my medical. Its a vacc. record as well as my one from my family physician. I am going to try calling her and see if she can help. Thanks.
