Medical Documents??

I hope everyone is in the best of health.

My husband's visa appointment is Nov 23rd. I just got another email from the embassy today with the instructions for his medical. It stated as follows:

Medical Examination

Immigrant visa applicants are required to have forms DS-2053, DS-3024, DS-3025 and DS-3026 filled out by a physician approved by the consular section. The following physicians have been approved by Embassy Tunis

My question is...are they supposed to provide me with these forms? or does the doctor have them?

Our Timeline

April 4, 2009: Married in Tunisia

June 2009: USC received residence card

October 20, 2009: Submitted I-130 at embassy (Tunis, Tunisia)

October 26, 2009: Received Packet #3 in the mail

November 1, 2009: Sent back packet #3

November 13, 2009: Received email from embassy notifying of final visa appointment scheduled for November 23, 2009

November 23, 2009: Appointment for visa. Accepted with NO INTERVIEW!!!

November 24, 2009: Took visa in hand from the Embassy!

December 9, 2009- we both fly to AMERICA!!!!!!! (inshallah)

The doctor will have the forms that you need. I would call ASAP for an appointment if you haven't already as there are few civil surgeons in Tunisia and appointments book quickly (many of them also serve other embassies ; Canada, France, Italy, etc) Verify when the appointment is made (best to have your husband call for clear communication in Arabic is easiest) that they have the forms on hand. Typically the doctor will fill out the forms and send them directly to the embassy OR give your husband a sealed brown envelope with all the medical forms inside that he can take to his interview. Also be sure that he has money for the blood work and any vaccinations they may send him to the pharmacy to buy. They will give you a cost for their services but some neglect to include the cost for bloodwork/lab/xray and vaccinations that he may need to receive.

I just wanted to add that while I think those forms you have mentioned will be available from the civil surgeon's office, you will need to take your appointment letter to the medical exam with you. That 4 page letter which you already should have received states your husband's name, case number, interview date, vaccination requirements, and information regarding what documents must be brought to your medical exam in Tunis.
