First of all.. Please delete my previous post of this at "Philippine Section" :P

...and uhmmm I would like to ask... :blush:

under DS-156 form question

#2 Place of Issuance?

* Do I have to leave the "Country" and "Province" blank? and just answer the City? as stated here => http://manila.usembassy.gov/wwwhevaf.html

#20 Name and Address of Present Employer or School ..

*coz currently I dont have a job and I am a College Graduate so what will I put in the blank?

# 22. When Do You Intend To Arrive In The U.S.?

(Provide specific date if known)

*I don't know yet so what will I put? even a tentative month, I cannot specify it, coz I not sure when..so whats the best thing I will put?

do I just leave it as blank ? coz its a drop down menu on months and you have to type the date and year...

and comparing the people's post here with regards to answering this form it confuses me whether to write NONE/N/A in the blank if I don't know the answer or if I don't have what the question is asking..

#2 I believe is referring to the issuance of your passport. If it's issued in the Philippines, put Philippines and the province as stated in your passport.

#8 and #9 Other Names, it says in the link you have posted to leave it blank if there's no other names so it should be fine. Though I put NONE on mine and I didn't encounter a problem.

#20 Name and Address of Present Employer or School - You don't have an employer nor a school you're attending now so I think what you should put is NONE.

#22 When Do You Intend To Arrive In The U.S.? - You can leave it blank or you can put any date you're planning to arrive in US.
