Immunization supplement form for I-693???

Hi,

Finally done with my Green Card Interview, everything smooth and no problem at all, the questions is quite easy actually. However they said I need Immunization supplement page to complite my green card.

I browsing around to find the Immunization supplement I-693, but I cannot find it.

- Any body know where I can find this immunization supplement form for I-693?

- It is complitely diffrent form then I-693?

I show on form I-693 seem to be more to be medical examination form, so where I can find the immunization supplement form only??? (Please help, kind a confuse)

The immunisation suppliment is I-693A, and is only available from a civil surgeon I believe. You will need to go visit a surgeon with your immunisation records and get it filled in. It will cost you money unfortunately.

Call around. The prices vary greatly.

You only need the I 893A vacination suppliment. Many of these surgeons think that they must do the full medical. Find one that will do just the suppliment.

They will need your DS 3025 shot record. If there are more shots necessary they will have to administer them or see some documentation where you have gotten them.

The civil surgeon will have it but the CDC has it too.

Funnily enough James and I are going today to get the I-693A (which is what you need) from a civil surgeon. All you need is to bring your DS-3025 to them and they will translate it onto the I-693A. The lady wasn't sure how much it would cost but guessed around $15-20, so not too terrible. However, I'm sure prices vary from place to place.
